About the Job:

PURPOSE STATEMENT:

The Pharmacist is responsible for maintaining pharmaceutical dispensing functions, including clinical review functions, in accordance with all legal and regulatory requirements. Provides pharmaceutical care and products to all patients. Dispenses medications; interprets and/or /evaluates medication orders; receives medications and supplies; and performs other functions as necessary to serve the hospital.

Roles and Responsibilities:

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

· Graduate degree from an accredited School of Pharmacy. Doctor of Pharmacy (PharmD) degree preferred.

· Minimum experience: hospital internship rotation.

LICENSES/DESIGNATIONS/CERTIFICATIONS:

· Active and in good-standing state Pharmacy license.

· Certification by the National Certification Board for Pharmacists and any required licensure.

ESSENTIAL FUNCTIONS:

· Interprets and evaluates medication orders (verbal and written) for appropriateness of therapy, incompatibilities, dosage, and age and transcribes to computerized patient mediation profiles. Maintains patient medication profiles.

· Seeks clarification on incomplete medication orders and obtains indications.

· Dispenses pharmaceuticals and controlled substances to patient care areas in a safe and timely manner.

· Maintains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection.

· Provides direction to pharmacy staff and ensures effective operation of the pharmacy department. Verifies daily activities of pharmacy technicians, interns and residents.

· Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures and protocols.

· Detects and reports suspected adverse drug reactions and medication occurrences.

· Utilizes formulary initiatives and clinical review to provide adequate pain management for the patient.

· Utilizes principles of drug dosing in the pediatric patient to provide medications to patients in correct doses and formulations.

· Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.

· Adjusts dosages or choice of medications based on decreased renal clearances in the elderly or other patients with compromised renal function.

· Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed.

· Participates in the review of opiate medications that are prescribed for appropriateness.

· Reads extracts and interprets information in patient charts.

· Provides clinical consultations and clarification to practitioners. Suggests appropriate cost-effective therapeutic alternatives to medical staff.

· Provides drug information to the professional staff, patients and families.

· Performs initial screening of order entry to provide information to perform interventions and documents all clinical activities and interventions.

· Monitors provision of off-hours pharmacy operations, stock medications, controlled substances, medications brought from home, and patient education.

· Conducts regular rounds on the nursing units to assess for compliance with requirements for medication control and storage.

· Participates in the development and presentation of orientation, education, and training programs to the pharmacy, medical, nursing, and other staffs.

· Performs monthly and biennial controlled substance audits.

· Assists with pharmacokinetics consult service and drug dosing per hospital protocol.

· Prepares annual evaluation of the Medication Management program, annual review and approval of the hospital formulary, as well as lists of Look Alike Sound Alike Medications and High Alert Medications.

· CPR and de-escalation/restraint certification may be required based on state requirements or facility policy.
