Job Description:

Provides leadership and has overall accountability for home health services. Participates in and is responsible for strategic planning decisions; Determines goals and objectives and responsible for assignments related to achievement of established organizational goals and objectives, work is tied to Post Acute Scorecard corporate initiatives. Subject matter expert on business line and resource in regards to policies, processes and operational issues as they impact the business line. Provides general direction and ensures managers and supervisors are developed and delegated authority to carry out daily operations.

Key Responsibilities include assuring regulatory compliance and monitor changes to policies
Implementation of Digital Services to support Value Based Care, reducing length of stay metric, reduce return to hospital metric, and reducing ER visits. The Home Health Director supports leadership in each office regarding operation of the program; management of activities, formulation of program goals and objectives; development and review of related policy; interpretation and administration of pertinent laws; preparation of budget and support of sales
Expansion of services to support our Essentia Health footprint.

Education Qualifications:

  • Bachelor’s degree in nursing, business, healthcare administration or related field required.
  • Master’s degree in nursing, business, healthcare administration or related field preferred.

Licensure/Certification Qualifications:

Certification/Licensure Required:

  • MN RN licensure upon hire.
  • Ability to obtain additional state licensures within 6 months of hire.
  • Requires minimum 5 years home health operations experience and an additional 3 years leadership experience.
  • Experience in Digital Health Services preferred.
