POSITION SUMMARY

Knows, understands, incorporates and demonstrates the mission, vision and values of Trinity Health in leadership behaviors, practices and decisions.

Develops and maintains relationships with all targeted health systems, hospitals, physician/at-risk organizations and post-acute providers (home health agencies, skilled nursing facilities, inpatient rehabilitation facilities, long term acute care hospitals).

Works with internal departments to generate ideas and develop and prioritize joint strategies for business and/or technology enhancements that will support patient outcomes, providers and attainment of financial performance metrics.

Maintains a working knowledge of applicable Federal, State and local laws/regulations; the Trinity Health Integrity and Compliance Program and Code of Conduct; as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.

MINIMUM QUALIFICATIONS

Graduation from accredited nursing program, current nursing licensure in the state where working. Bachelor of Science Nursing (BSN) preferred with 5 years of clinical and/or operational experience; 3 years of which are in acute care and/or skilled nursing facility, home health or other post-acute environment.
