The primary purpose is to provide each assigned patient with routine daily nursing care and services in accordance with the patient’s assessment, plan of care and care plan, as may be directed by supervisors.MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Must have previously completed a nurse’s aid training and competency program and must have previously passed a required competency test approved by the WV State Health Department (per Federal Long Term Care Regulation).
2. High school education or GED required.
EXPERIENCE:
1. One year nursing assistant experience in long term care preferred.
CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. They are not intended to be constructed as an exhaustive list of all responsibilities and duties. Other duties may be assigned.
1. Record all entries on flow sheets, notes, charts, etc. in an informative and descriptive manner per Policy/Procedures and assures they are correct.
2. Measure and record temperatures, pulse, and respirations per Policy/Procedures.
3. Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, communication, safety, education level and related criteria for the adult geriatric patient per the established age-specific standard.
4. Demonstrates the ability to use the following equipment according to manual: vital signs instrumentation, k-pad, scales, oxygen tanks, drainage systems/suction, SCDs, CPM machine, and Bladder Scanner.
5. Assist patients with daily hygiene (dental care, mouth care, bath, hair care, backrubs, nail care, and shaving, etc.
6. Assist patients with nail care, which includes cleaning and clipping, except diabetic patients per policy/procedure.
7. Turn and position bedfast patient every 2 hours and as needed. Assist patients with bowel and/or bladder functions. Assist patients in Bladder maintenance and bladder or bowel retraining programs.
8. Answer call bells promptly per policy /Procedure.
9. Assist with the care of the dying patient. Provide postmortem care per Policy/Procedure.
10. Provide perineal and indwelling catheter care on your shift and as necessary per Policy/Procedure.
11. Assist patients with (ADL) activities of daily living skills with and without adaptive equipment as per instructions of therapist or nurse.
12. Serve food trays per standard. Assist with feeding as indicated. Record food and fluid intake. Assist patients with identifying food arrangements with sight problem of foods or his/her tray.
13. Serve between meal and bedtime snacks.
14. Assist with lifting, turning, moving, positioning, and transporting patients into and out of bed, chairs, bath, wheelchairs, etc. utilizing proper body mechanics.
15. Report all allegations of patient abuse and/or misappropriation of patient property as per federal requirements and per policy/procedure.
16. Report all accidents and incidents you observe on the shift that they occur per Policy/Procedure.
17. Assists patient with specialized feeding techniques as per recommendations of the speech therapist in accordance with policy and procedure.
18. Assist with ambulation, gait training and CPM administration as instructed by the physical therapist.
19. Apply orthopedic devices such as AF orthosis, abduction braces, hinge joint immobilizer per instruction of the physical therapist.
20. Use safety equipment and supplies (mechanical lifts, steady’s, etc.) when lifting or moving patients who requires more than one person assistance.

1. Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling are necessary body movements utilized in performing nursing duties throughout the work shift.
2. Must be able to lift, move, and position patients of all weights, with varying amounts of assistance in a fair manner on a regular and recurring basis.
3. Hearing within normal range is required. (i.e.: to assess breath sounds, bowel sounds, apical pulse, monitors, etc.)
4. Visual acuity must be within normal range.
5. Must be able to perform heavy work: Exerting in excess of 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects
