Job Summary
Responsibilities
Under the direction of a faculty sponsor and in collaboration with the program support team, the APP coordinates the care of patients through the disease process and acts as a clinical liaison for patients, across medical services and clinical departments.
The APP evaluates and manages patient symptoms before during and after treatment.
The Advanced Practice Provider will provide comprehensive care to patients for the Section of Hematology/Oncology in The Department of Medicine.
Work collaboratively within a multidisciplinary team, using advanced clinical skills to manage a caseload of patients.
Assess, diagnose, initiate delegated medical treatments, perform independent nursing interventions and evaluate care.
Provide a model of care that is shaped by theory, expanded by science, research and reflective practice and is identified by a holistic and comprehensive approach to the patients.

Manage the care of patients, including assessment, diagnosis and treatment for assigned patients.
Document history and physical examination and changes in management according to government guidelines in the outpatient clinic at University of Chicago.
Coordinate diagnostic tests, follow-up on tests, and other patient care issues.
As part of duties, perform reporting for government agencies, chart review, refill prescriptions, and answer phone calls for patients for whom the APP is involved with longitudinal care.
Attend weekly conferences and yearly continuing education courses to keep abreast of hematology/oncology patient management.
Evaluates goals and outcomes taking into account the benefit-versus-burden, safety, quality, and cost-effectiveness for the patient, family and the organization.
Develops diagnostic strategies and therapeutic interventions needed to achieve the goals and outcomes of the patient’s plan of care.
Performs other related work as needed.

Preferred Competencies
Demonstrated clinical competency, effective leadership and teaching skills.
Ability to conduct physical examinations and health assessments.
Ability to interview and obtain medical information.
Willingness to enter into written collaborative agreement with a collaborating physician upon hire.
Once licensed and as needed, willingness to apply for a mid-level practitioner controlled substance license and DEA number.
Willingness to be privileged and credentialed through UCM Medical Staff Office prior to start date.
Strong interpersonal skills and the ability to work independently.
Strong organizational skills.
Demonstrated written and verbal communication skills and strong analytical skills.
Proficiency in the use of computers.
