We are looking for Casual Registered Nurses who will provide clinical leadership in the delivery of individualised care to residents, enhancing their health and spiritual well-being. You will be assisting residents in the comfort of their own homes and providing quality care as a Registered Nurse in Home Care.


ABOUT THE ROLE

In the role of Registered Nurse in Home Care, you will provide clinical supervision and be responsible for coordinating the delivery of safe, effective and individualised care to our care recipients and their families (including managing staff and physical resources) and assisting residents to lead an active and meaningful life as independently as possible in keeping with The Salvation Army's Mission, Vision and Values. 

  • As a Registered Nurse with The Salvation Army Aged Care you will:
  • Deliver comprehensive, best-practice clinical nursing care to SHC clients, conducting assessments, developing, and monitoring care plans, implementing, and evaluating care to professional nursing and Aged Care accreditation standards.
  • In collaboration with Care Coordinators, liaise with medical and allied health professionals, ensuring timely referral, feedback, and follow-up of clinical risks and concerns. 
  • Maintain effective care planning and reporting documentation of client care, identified risks, incidents and management of high-risk client behaviour or changes, in accordance with SHC policies, procedures and systems. 
  • Participate in case conferences and coordinate client nursing are, liaising with Care Coordinators to ensure effective allocation of staffing resources and funding to promptly address client care needs. 
  • Collaborate with and support internal and external stakeholders, including after-hours on-call staff as required to ensure timely and effective management of client care needs and issues.  
  • Supervise, support and provide feedback to assist in the development of Home Care Workers to ensure safe, effective work practices and high standards of service delivery.  
  • Contribute as a team member and clinical leader to the development of policies and procedures, quality improvement processes, program innovations and projects, providing relevant client and program data. 
  • Actively participate in professional supervision with the Clinical Coordinator, training and other continuous professional development activities to maintain AHPRA registration and ensure currency of clinical knowledge and practice in line with TSA mission, values and policies. 

ABOUT YOU 

To be considered for this role, ideally we are looking for: 

  • An understanding, empathy, appreciation and the ability to promote the values and ethos of The Salvation Army. 
  • Current unrestricted registration with Australian Health Practitioners Regulation Agency 
  • Demonstrated aged care experience, the ability to work as part of a team and promote education and leadership in aged care 
  • Understanding of and commitment to continuous quality improvement 
  • Experience in developing care plans 
  • An understanding of Aged Care Quality Standards 
  • An understanding of AN-ACC
  • Demonstrated organisational and time management skills 
  • Demonstrated well developed computer and documentation skills 
  • National NDIS worker screening check
