Key/Essential Functions & Responsibilities
Wound Care, Medication Management, G-Tube Administration, etc.
Report any inconsistencies in our member’s condition and family situation
Document reports and observations
Interacts with patients and families
Teach appropriate self-care techniques
Consult with Case Manager/MD in

Maintains reporting systems
Report abuse or unsafe/illegal practices
Make recommendations to improve quality of life for member
Minimum Qualification Requirements including education, experience, licensure/certification, etc. and essential physical capabilities (e.g. lifting, assisting lifting, standing, etc.) Minimum requirements of specific positions may vary by assigned function and/or contract. All LPNs must meet the following minimum requirements:
NYS Licensed Practical Nurse (LPN) License
Preferred Qualification Requirements (desired requirements beyond MQRs above)
Med/Surg experience
Experience with intellectually and developmentally disabled population
Compensation:
$63,000.00
Schedule:
Mon, Wed, Fri, every 3rd Sat 7am-7pm
Tue, Thu, Sun, every 3rd Sat 7am-7pm
