The Occupational Therapist plans, evaluates, and provides Occupational Therapy Services to homebound patients to improve function, increase independence with ADL's, retrains and instructs in compensatory techniques for psychological, sensory, physical, and cognitive deficits, and preventative rehabilitation programs.  May function as primary case manager when Occupational Therapy is the primary service provider. OTRLs/OTLs in this position perform their regularly assigned tasks as members of a geographic or specialty team under the direction and leadership of the HH Clinical Services Manager. Under general supervision and in accordance with the policies, procedures and guidelines established within the organization, the unit, and the Michigan Public Health Code, the Occupational Therapist manages the care delivery for patients to achieve quality outcomes by the following: Evaluates the Occupational Therapy patient to determine the rehabilitation needs and potential for achievement and develops a plan of care in accordance with the findings. Makes the initial therapy evaluation visit and re-evaluates the patient's occupational therapy needs during each visit. Makes follow-up assessments according to OASIS and PPS regulatory guidelines. Communicates significant findings, problems, and changes in condition or environment to the appropriate supervisor, the physician, and other team members involved with patient care. May initiate plan for patient safety, using the patient, family, and community resources. Participates in the implementation of and development of the Plan of Care to ensure quality and continuity of care and proper discharge planning. Verifies the Plan of Care prior to each visit and provides care according to physician's orders, assessment data, and established standards and guidelines. Initiates and revises the Plan of Care in response to identified patient care issues. Writes physician orders to cover additional visits and changes to the Plan of Care, per agency policy. Incorporates patient care goals established in the Plan of Care, into therapy care, as evidenced by documentation in therapy notes. Performs direct therapy services in accordance with accepted standards of practice and certified by the patient's physician. Selects diagnostic and therapeutic materials appropriate to patient needs. Instructs and provides patients and/or their caregiver in the home therapy program providing modification and clarification when necessary. Instructs and evaluates the Home Health Aide with special patient care procedures in accordance with the Home Health Aide scope of practice and home health care policies. Instruct and evaluates the therapy assistant, if applicable, with the patients Plan of Care in accordance with the therapy assistant scope of practice and home health care policies. A degree from an accredited Occupational Therapy curriculum.   Currently licensed as an Occupational Therapist in the State of Michigan. Must have and maintain active BLS.   Current Michigan Driver's License. Proof of Auto Insurance.   Required 32 hours per month - may include 1 weekend shift.   Mental & Visual Fatigue: Work which produces high levels of mental/visual fatigue, e.g., interactive and repetitive or small detailed work requiring alertness and concentration for sustained periods of time, the operation of and full attention to a personal computer or CRT between 40 and 70 percent of the time. Physical Effort: Involves regularly lifting of bulky or moderately heavy weights (i.e., up to 50 pounds) and occasionally assisting with heavier tasks or expending the equivalent effort in pushing, pulling, or otherwise handling material, equipment and other objects. Life Support Certification Required: BLS
