Job Description
As an integral part of The Chicago Institute for Fetal Health (CIFH), the Fetal Nurse Coordinator serves as the main clinical point of contact for pregnant patients diagnosed with fetal conditions. The coordinator ensures longitudinal care coordination across institutions and specialties. The position reports to the CIFH Nursing Director.
The IFH RN Coordinator will work 4 10 hour shifts M-F .
One week of on call rotation required every 3-4 weeks, which includes weekends and holidays.
Five years of Labor and Delivery, MFM, NICU, or Obstetrics RN experience is required for an interview.
Essential Job Functions:
1. Pre-Visit
a. Corresponds with patients for pre-visit intake, education, and preparation for visit
b. Reviews outside records and ensures pertinent clinical information is received
c. Reviews pertinent history with Medical Director and collaborates in developing plan of care
d. Coordinates multidisciplinary consults, imaging, and tests
2. Day of Visit
a. Meets patient and family and provides education and preparation for visit
b. Collects and assimilates pertinent imaging and clinical data to provide at multidisciplinary team meeting pre-huddle
c. Participates in multidisciplinary team meetings involving subspecialty physicians, nurses, social worker, and genetics counselor
3. Post-Visit
a. Coordinates follow-up appointments, tests, and delivery planning meetings
b. Prepares care coordination notes, communicates care plan, and corresponds with referring providers (nurses and physicians)
c. Reviews follow up OB and MFM records (fetal echo, level II ultrasound, genetic tests, etc) and provides ongoing updates to fetal surgeons, physicians and other multidisciplinary team members, as indicated
4. Care Coordination
a. Arranges meetings with specialties including Maternal Fetal Medicine, Neonatology, Pediatric Surgery, and a variety of Pediatric subspecialists within Lurie Children’s Hospital
b. Corresponds with patients prior to meeting with the care team to address questions and concerns
c. Maintains ongoing contact with patients and families, as needed, to answer questions and provide care guidance
d. Coordinates transfers of care with other institutions as needed
5. General
a. Develops in depth knowledge of various perinatal and neonatal conditions and their evaluation and potential treatment plans.
b. Serves as primary liaison for community OB and MFM specialists, and provides thorough communication with providers regarding their referred patients
c. Coordinates and participates in pre-op, intra-op and post-op care of patients undergoing fetal surgery
d. Works closely with social workers and genetic counselors to coordinate testing and care needs for patients and families
e. Leads daily fetal team huddles that focus on new patients and scheduling processes
f. Prepares reports and patient lists utilized for quality management purposes
g. Actively participates in CIFH and subspecialty education and business meetings
h. Takes on call with the nursing team to cover patient care needs, on a rotational basis
i. Participates in CIFH research efforts, including the collection of clinical and research data
j. Develops and updates family/patient education materials
k. Leads projects and committees within CIFH based on goals and needs identified
l. Other job functions as assigned
Minimum Qualifications:
1. A BSN from an accredited school of nursing and current licensure as a registered professional nurse in the State of Illinois required. Master’s degree from an accredited nursing program preferred
2. Minimum of 5 years clinical nursing experience in one or more relevant fields required: Labor & Delivery, Obstetrics, Maternal Fetal Medicine, or Neonatal Intensive Care (Labor and Delivery experience preferred)
3. Experience working in partnership with health care professionals and support staff in a multidisciplinary setting required
4. Excellent organizational skills, including ability to effectively manage multiple competing responsibilities, required
5. Proficiency with Microsoft Office and standard office equipment required
6. Electronic medical record experience required. EPIC experience preferred
7. Superb written and verbal communications with patients, families, physicians, and staff required
8. Possesses a high level of initiative and autonomy.
9. Demonstrates integrity and accountability
10. Basic Life Support certification required
