Provides and promotes a proficient level of nursing practice to efficiently provide safe, effective, and holistic patient and family centered care for the patients and families with complex needs. Uses increased knowledge, skills, and experience to understand situations holistically and plans and adjusts accordingly. Quickly recognizes variations from the anticipated course, thinks critically, modifies plans, makes appropriate decisions, recognizes salient patient care issues, and focuses on long term goals.

Description
1. Demonstrates and promotes excellence in the application of the AC Model of Patient and Family-Centered care. Collaborates and role models partnerships with patients, families and other providers to plan, deliver, and evaluate highly individualized care that ensures achievement of effective self-care by patients and families. Consistently plans and delivers high quality, comprehensive and holistic care for complex patient situations. Anticipates needs across the care continuum for patients and families and acts as a resource to others in doing the same.
2. Consistently provides safe and effective nursing care at a proficient level. Role models and promotes practice which is current and consistently in accordance with policies and procedures, professional practice standards, state Practice Act/Rules and Regulations. Demonstrates proficient technical and procedural nursing skills. Role models and promotes effective interdisciplinary communication to assure continuity of care to achieve desired patient outcomes.
3. Proficiently assesses increasingly complex patients and serves as a resource to others to ensure developmentally appropriate and culturally sensitive care. Anticipates, incorporates, and promotes the use of appropriate diversional, play, or recreational activities into patient care.
4. Analyzes increasingly complex patient/family information to identify problems and outcomes, evaluates complex variations and patterns in patient care data for clinical implications.
