Job Title: Case Manager RN CCHP
Hours: Part-Times) The position is remote; however you must be local as we do ask staff to come in for team meetings.
What you will do:
Supports Children's Community Health Plan (CCHP) members with chronic health conditions utilizing an integrated, comprehensive approach to care. Assists in the direct and indirect coordination of care through member, caregiver, practitioner and community education while addressing medical, behavioral, educational and social needs.
Bachelor's degree in Nursing (BSN) required within 1 year of hire
Registered nurse with a valid, unrestricted Wisconsin nursing license.
Requires a valid driver's license, acceptable driving record, personal auto liability insurance coverage and the ability to be insured by the CHHS auto insurance carrier.
Two or more years of nursing experience. Experience in a Medicaid HMO/managed care or public health setting preferred.
Strong organizational skills.
Demonstrated ability to prioritize tasks and handle multiple issues while meeting timelines.
Excellent oral and written communication skills.
Ability to communicate effectively and work collaboratively with internal departments, external providers and members.
Demonstrates teamwork within the department and community by assisting other staff members and/or organizations as necessary.
Independent judgment, analytical ability, systems thinking and interpersonal communications to work in complex systems.
Awareness of community organizations and resources to address member and family needs.
Computer knowledge and skills in word processing required.
