We are seeking a dynamic and dedicated  Licensed Practical Nurse - Panel Manager (limited duration) to join our team at the Mid County Health Clinic and lead the charge in providing high-quality, affordable care to our community.  Integrated Clinical Services (ICS) is the largest Federally Qualified Health Center (FQHC) in Multnomah County, providing quality health services for people who experience barriers to accessing care. About the position:

The Licensed Practical Nurse (LPN) works within the health care team and provides focused community and clinic health services directed towards the prevention of disease, the promotion of health, the reduction of disability, improving the clients self-care, using professional judgment in focused/limited assessments, screening, contributing to care planning, intervention, and provides health information/education to patients. The Panel Manager role tracks the health of the population of patient's assigned to the provider panel. Manages team communication including phone messages, fax requests, and warm hand-offs, as needed. The Panel Manager is responsible for the team InBasket within the electronic health record system and delegates requests to appropriate staff, in addition to completing those requests that are within the scope of a LPN. The Panel Manager completes needed patient visits as noted on EPIC team schedule in addition to providing focused assessments to walk-in clients and supports providers when staffing indicates the need. Creates a work culture of respect, trust and understanding for the highly diverse populations the Health Department serves in the office and in the community. This is a Limited Duration position assignment.

What You’ll Do:

  • Direct Client Care: This includes health education on various topics (prenatal care, chronic disease management, navigating the US healthcare system, etc.), focused assessments, and assisting with procedures (immunizations, wound care, etc.).

  • Care Coordination: The role involves significant care coordination such as scheduling appointments, managing referrals, communicating with providers and the care team, and following up on client needs.

  • Panel Management: This involves proactive client management using electronic reports to identify needs for preventative screenings, chronic disease monitoring, and immunization forecasting.

  • Team Leadership: The position requires leading and directing staff to ensure smooth clinic flow.

  • Administrative Tasks: Duties include managing communication channels (phone, in-basket), patient reminders, and meticulous documentation in patient charts.

Minimum Qualifications/Transferable Skills*: Any combination of experience and training that would likely provide the knowledge and abilities is qualifying.   • An Associate's degree or a Bachelor's degree in nursing from an accredited nursing program

  • LPN Licensure

  • BLS CPR Certification

  • A minimum one (1) year of nursing experience
