Position Summary

Responsible for providing individualized assistance across the care continuum to patients, families, and caregivers to expedite, coordinate care, and address health system barriers. Assumes the responsibility for the coordination of patient care through interdisciplinary and multidisciplinary collaboration to achieve optimal patient outcomes with a focus on high-risk patients or those with complex care needs (including multidisciplinary care) while building and maintaining community relationships to provide expedient and reliable access to quality patient care.

Job Description

Primary Duties & Responsibilities:

  • Coordinates patient services with physicians and staff to provide quality care in an appropriate and effective manner; provides appropriate counsel and teaching to patients and families; ensures continuity of care and promotes health care maintenance. Bridges gaps in care, and assesses/troubleshoots care transitions and barriers to care. If order entry in the electronic medical record is part of the job function, completes requisitions/orders per Washington University guidelines.
  • Promotes and maintains good public relations with patients/families and acts as a liaison between other departments.
  • Assists physicians with examinations, procedures, education and other processes related to direct patient care. Provides direct patient care, including administering medication, vitals, and phlebotomy, monitoring IV therapies, and maintaining records in accordance with approved nursing techniques.
  • May participate in clinical research; assists physicians and other health care providers in completing data for research studies and provides documentation required for studies. Enhances access to therapeutic clinical trials and optimizes communication with patients, families and investigators regarding therapeutic clinical trials, including patient-specific education about clinical trials.
  • Participates in continued professional development, attending educational programs, in-service, and meetings as needed.
  • Maintains navigation services documentation according to program standards.
  • Performs other duties when needed.

    • Graduate of an accredited nursing program with 3 years of nursing experience.

    Salary Range

    $60,800.00 - $105,700.00 / Annually

    The salary range reflects base salaries paid for positions in a given job grade across the University. Individual rates within the range will be determined by factors including one's qualifications and performance, equity with others in the department, market rates for positions within the same grade and department budget.
