LOCATION: 330 Monroe Ave. Rochester, NY
DEPARTMENT: Home Care/Home Care Hospice
SCHEDULE: Monday-Friday, Days
ATTRIBUTES
Associates Degree required, Bachelors and/or Masters Degree in Nursing preferred
Current RN licensure in the State of New York required
Minimum of 2 years RN nursing experience, at least 1 year of which must be in home health care
RESPONSIBILITIES
Facilitation. Facilitates the orientation process for all new home health staff. Develops, organizes and delivers all classroom or virtual orientation lesson plans. Facilitates and/or develops and deliver continuing educational opportunities for staff.
Assessment. Assesses staff competency levels and facilitates various methods to accomplish competency completion. Provides ongoing educational needs assessment to identify relevant educational activities such as new diagnoses, new equipment, policy and procedure expectations, and quality outcome data. Determine effectiveness of educational offerings and analyze cost to maximize the efficient utilization of resources.

Utilizes a variety of learning methods such as power point presentations, on-line computer programs, self-learning packets, demo/re-demonstration, simulation training. Utilizes effective communication strategies by developing communications and audio visual materials that are clear and concise.
Education. In collaboration with Directors/Managers, provides support and education remediation, as appropriate. Maintains strict record keeping of all educational activities with attendance as required by regulatory agencies such as NYSDOH.

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.

PAY RANGE:
$79,560.00 - $109,200.00CITY:

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
