Kaweah Health approved crisis intervention training within 60 days of hire/transfer.

Experience
Required: Five years of management experience in nursing leadership.
JOB RESPONSIBILITIES
Essential
The Director of Post Acute Nursing provides daily and strategic administrative oversight and operational control for acute rehabilitation and skilled nursing services. This position requires the Director to provide management, administrative, and financial supervision of the delivery of care for acute rehabilitation and skilled nursing services.
Directs the delivery of patient care facilitating practice that is in the alignment with all regulatory requirements. Acts as the Administrator of Skilled Services.
Develops patient care policies, procedures, protocols and standards which guide and support the provision of services.
Holds direct reports accountable for meeting standards of nursing practice and patient care.
Develops, implements, evaluates and, as necessary, revises the plan for the provision of care and service in rehabilitation and skilled nursing units. Contributes to, and coordinates services with, the development, implementation and evaluation for the provision of care and services.
Assures that the facility receives no preventable significant citations from CDPH.
Provides adequate staff resources for carrying out the plan for patient care.
Demonstrates knowledge of both federal and state requirements for acute rehabilitation and distinct part skilled nursing facilities.
Identifies patients at risk for abuse and/or neglect, as well as associated signs and symptoms. Responsible for the management of an effective preventative abuse and neglect program. Adheres to mandatory reporting requirements for healthcare professionals per Administrative Policy & Procedure.
Maintains effective working relationship between Kaweah Health employees and medical staff by ensuring a consistency of purpose and mutuality of interest between the operations of the services, the bylaws, rules and regulations of the medical staff. Provides support to medical staff as appropriate.
Demonstrates the knowledge and skills to safely and appropriately fulfill job specific responsibilities related to safety, security, emergency preparedness, control of hazardous materials/waste, life safety and medical equipment as outlined in the environment of care plan for the Rehabilitation Hospital and the South Campus. Observes safety precautions to ensure a safe environment for patients, patient families and guests of all ages.
Integrates services with the Kaweah Health's primary functions.
Determines the qualifications and competence level for department staff. Makes recommendations regarding required staffing levels.
Enhances professional growth and maintains awareness of current industry trends, legislation or research which could impact department and the Kaweah Health.
Assures quality of care and maintains current knowledge of key regulatory and legal requirements. Continually appraises and responds to results of all Quality Assurance programs. Investigates and responds to physician, resident, family or employee concerns or complaints about the quality of care. Establishes and enforces a facility wide policy of resident's rights and responsibilities.
Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area. Knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs. Able to effectively communicate and care for patient and family as reflected in the Plan for Provision of Care.
Pay Range
