Community nursing is fulfilling, and every shift you’ll feel like you are making a difference for someone. If you’ve lost your 'spark' and want to get back to delivering personalized nursing care, this could be the opportunity for you.
We are looking for an experienced Registered Nurse to join our community team based in Arncliffe. As a Community Nurse, you will build rapport with your clients in their homes while providing care in line with their personalized care plans. This includes health assessments, chronic wound care, medication administration, and offering education and support to patients and their families.
In this role, you will work as a Primary Nurse, providing hands-on care to meet the needs of your patients, including:
- Complex wound management
- Palliative care support for patients and their carers
- Symptom management and end-of-life care
- Continence and catheter care
- Stoma care
- Support for the aged and those with chronic illnesses
You will liaise with the patient’s health and non-health team to ensure safety at home, providing education and support to enable independence and effective management of health conditions.
ABOUT YOU
To be considered for this role, ideally you will have:
- Current unrestricted registration with AHPRA as a Registered Nurse
- A current driver's license and reliable vehicle
- Demonstrated community nursing experience, delivering a broad range of nursing care and interventions
- Ability to work autonomously with a patient-centered approach
- Experience in developing care plans
- Understanding of Aged Care Quality Standards and AN-ACC
- Demonstrated organizational and time management skills
- Well-developed computer and documentation skills
- Experience in chronic wound care
