SUMMARY
The Hospice Liaison is responsible to facilitate the patient discharge from the facility setting to certified hospice agency. The Hospice Liaison will provide superior customer service by modeling the Brand Promise and Core Values.

STATUS: Full Time
LOCATION: 330 Monroe Ave. and Unity Hospital
DEPARTMENT: Home Care Hospice
SCHEDULE: Monday-Friday, 8:30am-5pm
ATTRIBUTES
Social Work, LPN, or RN Licensure required
• Ability to travel required
2 years hospital, home care, or hospice experience preferred
Experience in working in partnership with the interdisciplinary healthcare team
Excellent interpersonal written and verbal communication skills
Experience utilizing an Electronic Medical Record and Microsoft office
Demonstrated ability to apply decisive, sound, objective clinical judgment for cases with varying complexity
RESPONSIBILITIES
Evaluation & Collaboration. Evaluate eligibility, needs and care resources for patient; obtain insurance information and authorizations for appropriate reimbursement; collaborate with facility care coordinators, social workers and physicians to find solutions for challenging patients requiring home care
Assessment & Development. Complete a comprehensive clinical assessment for home care services and develop a home care plan; coordinate all in-home services, including professional, paraprofessional services, durable medical supplies and equipment
Documentation. Complete all required documentation in patient’s facility record; gather and analyze data and complete all assigned reports
EDUCATION:
AS: Nursing (Required)LICENSES / CERTIFICATIONS:
BLS - Basic Life Support - American Heart Association (AHA), RN - Registered Nurse - New York State Education Department (NYSED)PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.

PAY RANGE:

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
