Overview
Works under the supervision of the practice manager and the Director and is responsible for the assessment of PORT participants, on an intermittent visiting basis. Assessments will be used to facilitate early detection and management of health issues, reducing the burden on emergency services and hospitals. The RN may work in a variety of settings with a diverse patient population of all ages. The RN utilizes communication, critical thinking, problem solving, and decision making to create a care plan for each patient based on physiological, psychosocial and cultural diversity. Responsible for submitting referrals and coordinating transportation services for patients. Works under the direction of leadership and a multi-disciplinary team to provide access to a comprehensive range of services or clinical care associated with and in accordance with standards of excellence established by Riverside Health system and facility goals and strategic plans.

  • Demonstrates ability to assess patient clinical and psychosocial needs by applying standards of practice as it relates to the specialty or patient population served in the PORT.

  • Partners with patients and families to develop, implement and evaluate plan of care, treatments and procedures that contribute to the physiological and psychosocial outcomes of the patient. Assesses and incorporates evidence-based standards of practice. Collaborates with interdisciplinary team in the coordination care. Facilitates safe patient transitions to the next care setting as part of plan of care.
  • Demonstrates a sense of urgency to facilitate patient throughput to assure patients receive consistent, competent, and ethical care. Focuses on quality outcomes of patient care when addressing guest needs to include current evidence based practice and quality measures.
  • Demonstrates understanding of diverse patient care populations and ability to modify education method to ensure patient/family comprehension. Educates by assuming responsibility to individualize and coordinate patient education, and discharge specific education that includes family/caregiver. Evaluates for patient comprehension and documents.
  • Ability to delegate and prioritize tasks appropriately to facilitate quality patient care and ensure patient care needs are met. Able to recognize and respond to emergency situations. Evaluates own needs for support and uses identified systems to meet need.
  • Accurately completes all required clinical and quality documentation consistently per RHS standards and regulatory requirements.

Qualifications

Education
  • Associates Degree, Registered Nurse (Required) or
  • Bachelors Degree, Registered Nurse (Preferred)

Experience
  • 2 years Registered Nurse (Required)

Licenses and Certifications
  • Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) Upon Hire(Required)
  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) Upon Hire(Required)
