JOB SUMMARYFacilitates and coordinates the evaluation of potential transplant and/or living donor candidates. Assists with coordination of care and discharge process for recipients hospitalized for transplantation and living donation and post-operative complications. Provides clinical management transplant recipients and/or living donors, who are followed as outpatients, in collaboration with the transplant team (attending physicians, nurse practitioners, clinical transplant pharmacists, social workers).
The candidate selected will perform but not limited to the following:
- Management of post-transplant patients in the outpatient setting.
- Responsibilities include education of the patients and their caregivers prior to discharge from their initial hospitalization following surgery.
- Continued lifelong management of patient needs, including medication refills, following up on clinical needs from clinic visits, sick calls, and coordination of care for patients.
Preferred Education, Skills, and Experiences:
- Must have a BSN and 2 years of nursing experience.
- Cardiac experience or prior transplant experience is a bonus.

Hours:
This is a Full-Time position. DAYS. Monday - Friday 8:00 a.m. - 5:00 p.m. This is a hybrid position after the candidate selected is fully trained. Training will occur in the Nashville office for the first several weeks (approximately eight weeks or thereabouts).

Our transplant specialists also perform lifesaving dual-organ transplants, including heart-kidney, heart-liver, heart-lung, kidney-pancreas and liver-kidney.

KEY RESPONSIBILITIES
• Coordinates the evaluation process of potential transplant candidates and/or living donors in collaboration with physicians and other health care professionals.
• Educates patients and families about the transplant process, evaluation and waiting phases, and long-term follow-up and care, as well as, living donation when applicable.
• Monitors patient progress and provides clinical management during post-transplantation and post living donation.
• Ensures the most current practices of transplantation nursing are utilized, and providing system improvements to support optimal transplant outcomes.
• Ensures referring physician is kept informed about all phases of the transplant process.

TECHNICAL CAPABILITIES
• Transplant Coordination (Intermediate): Demonstrates mastery of transplant coordination in practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering coordination services without requiring support and instruction from others. Possess sufficient knowledge to provide interventions that maintain or improve physiology, psychological and social health and interventions that facilitate and optimize behavioral changes and treatment adherence with complex, lifelong therapies. Advocates supporting patients and families in planning, implementation and evaluation of care. Able to train and educate by setting the example, giving technical instruction, providing leadership, and generally raising the level of performance of others while on the job.
• Nursing Patient Assessment & Evaluation (Intermediate): Demonstrates mastery of patient assessments and evaluations in practical applications of a difficult nature. Conducts primary care patient interviews and physical examinations. Conducts inquiry with a thorough series of questions when patients state an ailment or complaint in order to accurately identify and elaborate on the problem. Possesses sufficient knowledge, training, and experience to role model and coach less experienced peers.
• Nursing Patient Education (Intermediate): Demonstrates mastery of patient education in practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to be capable of providing patient health education services without requiring instruction from others. Able to train and educate by setting the example, giving technical instruction, providing leadership, and generally raising the level of performance of others while on the job.
