Scheduled hours: Night shift. Five, 12-hour night shifts per pay period with weekend rotation.Job Description:
Coordinates patient care with input from the hospice interdisciplinary team. Communicates with insurance companies for authorization and approval for hospice services initially and ongoing through the course of care. Evaluates patients for eligibility for three levels of hospice care- respite, residential, and general inpatient care. Documents patient care to support each level of care for regulatory and payment purposes. Supervises and assigns care to hospice aides and LPNs. Prepares agenda and coordinates hospice interdisciplinary team meetings. Monitors hospice inpatient building maintenance needs and security after office hours. Collaborates, communicates, provides psychosocial support and education to family members or significant others. Delivers care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation in coordination with other health team members.
Qualifications:
Prefer Hospice and Palliative Care Nurse certification
Prefer minimum of one year acute care hospital experience
Registered Nurse licensed in the State of Iowa
Graduate of an accredited school of nursing
