We are seeking a committed Registered Nurse who will provide clinical leadership in the delivery of individualised care to residents, enhancing their health and spiritual well-being at our aged care centre.

Access Health provides accessible, responsive Primary Health Care that enhances the health and well-being for people who inject drugs, street sex work or experience homelessness. Access Health offers non-appointment based services that are free of cost and facilitates linkages to mainstream services. The program works in partnership with other health and support services to provide clinical and non-clinical health services and practical support. 

About the role

We are seeking a values-driven and proactive GP Practice Nurse who will be responsible for broad clinical activities with an emphasis on chronic disease management, preparing GP management plans, care plans and health assessments.

Reporting to the Program Manager, this is a Casual, full-time position based in St Kilda, VIC. 

You will successfully

  • Perform clinical duties. Duties include triage, immunisations and other injections; wound management, chronic disease management, ECG’s and collection of pathology samples.

  • Actively work to ensure effective team communication, when necessary facilitate communication between GP’s and duty desk to minimise patient wait time.

  • Conduct preventative/screening procedures; assist with patient education and community health promotion activities.

  • Co-ordinate patient recall and GP management plans and team care arrangement.

  • Act within the practice and nursing code of ethics & appropriate level of clinical competence at all times.

  • Maintain awareness of current evidence and research on clinical practices and inform/educate other practice staff.

  • Consistently be aware of OHS requirements and comply with them.

You will have

  • Current unrestricted registration with Australian Health Practitioners Regulation Agency.

  • Experience and competency in delivering accessible Primary Health Care, AOD and Harm Reduction Services. 

  • A strong understanding of the health and AOD harm reduction needs of people who use drugs, people who are experiencing homelessness, people who engage in street-based sex work or people experiencing discrimination, stigma or issues accessing health services due to other reasons. 

  • Excellent organisation and administrative skills with capacity to balance workload and determine priorities while also addressing the needs of a high volume, often crisis orientated environment. 

  • Strong verbal and written communication skills. 

  • Evidence of two doses of the COVID-19 vaccination.
