Overview
Provides advanced nursing clinical care to patients in the VNS Hospice Care program consistent with VNS Health philosophy and standards of nursing practice in relation to VNS Health goals and objectives and improves hospice utilization for hospice eligible patients. Works under general supervision.Compensation Range:$130,140.00 - $155,540.00 AnnualAlong with our highly competitive base pay, we offer pay differentials based on education, clinical experience, certifications, and work in high need areas.

What You Will Do
Practice independently in the community as part of an interdisciplinary care team.
Deliver personalized nursing and care management to patients in their home or care facilities.
Constantly evaluate evolving patient needs and respond with plan of care adjustments.
Qualifications
Licenses and Certifications:
- Licensed and currently registered to practice as Nurse Practitioner in New York State required
- DEA license required
- Valid driver's license may be required, as determined by operational/regional needs
Education:
- Master's Degree in Nursing required
Work Experience:
- Minimum of three years clinical nursing experience required
- Experience as a Nurse Practitioner preferred
