This is a great opportunity for an experienced Registered Nurse to work close to home at a 3-time Magnet -recognized community hospital. 

Job Profile Summary 

The Stroke Nurse Navigator (SNN) reports directly to the Stroke Program Manager, who works in close collaboration with the Director of the Stroke Center. The SNN serves as a point of contact for patients and families for Stroke Center. The SNN coordinates with multidisciplinary teams across the continuum and is involved in activities associated with the stroke program to ensure optimal transitions of care. The SNN will assist in data collection and performance improvement for the hospital’s stroke program. The SNN is responsible for supporting regulatory compliance with the Commonwealth of Massachusetts, The Joint Commission and the nationally recognized clinical practice guidelines as they pertain to Stroke Centers and care delivery. The SNN will work as part of a multidisciplinary team to provide education and guidance to patients with stroke. This clinical provider will provide patient care within regulations in accordance to their licensure within the Commonwealth of MA.

Responsibilities:

  • The SNN will perform daily rounds on inpatients with stroke as a primary diagnosis.

  • Supports the stroke program and maintains regulatory compliance with state and national standards for designated stroke centers.

  • Collaborates with medical and hospital staff to provide multidisciplinary care to stroke patients

  • In collaboration with the Stroke Program Manager and Stroke Medical Director, develops program plans, policies, protocols, and pathways to support timely and effective care for stroke patients

  • Collection and analyzes stroke program patient data to support documentation, clinical care, and improved outcomes

  • Collaborates with stakeholders to develop the necessary educational components and teaching material for staff, patients, and the community

  • Supports the development of performance improvement teams and/or coordinates with leadership

  • Ensures the protocols and therapies become the standard of care followed within the facility

  • Act as a member of the Stroke Activation Team providing guidance on processes and workflows. Serve as a leader in patient stroke education, working with bedside nurses, therapists (PT, OT, SLP), and the Neurology team to provide stroke education to patients and/or families

  • In conjunction with Case Management and social work, will assess caregiver readiness

  • Provides education and counsel patients and their family/support network on stroke care and services

  • Conduct follow up phone calls to improve transitions of care, patient safety, and patient satisfaction

  • Collaborate with Stroke Program Manager and community agencies to provide community outreach and education initiatives and stroke support groups

Hours:

40 hours/week

M-F day shift with occasional off shift and weekend hours for rounding, and education with nursing staff and any stroke community events

 

Minimum Qualifications: 

1. Associate's degree in Nursing. 

2. Registered Nurse (RN) license. 

3. Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification. 

4. Three (3) years clinical experience caring for patients with a broad range of complex medical diagnoses. 

 

Preferred Qualifications: 

1. Bachelor's degree in Nursing. 

2. Advanced Practice Registered Nurse (APRN) 

3. Eligible for state and federal controlled substances registration. 

4. Five (5) years clinical experience in home health, post-acute, or acute settings caring for patients for a broad range of complex medical diagnoses. 

5. Bilingual.
