About the role:

The Commonwealth Home Support Programme (CHSP) provides clinical nursing care, allied health and specialist support services to clients who live and/or work in the Local Government Areas of Waverley, Woollahra, and the City of Sydney.

The Nurse Unit Manager will be responsible for the day to day operations and supporting staff to deliver person centered care for patients who are accessing Commonwealth Home Support Program (CHSP) services.

The Nursing Unit Manager for the CHSP will be accountable for the management of referrals and waitlists of the CHSP service.

The CHSP NUM will ensure that the CHSP service meets the required KPIs by the local organization, the Commonwealth and the Ministry of Health, whilst supporting staff in the delivery of care to patients accessing Aged Care Services.

About you:

You will be expected to be accountable for the operational management of the St Vincent’s CHSP service. You will ensure that the CHSP service meets the KPIs required by the local Organisation and the Ministry of Health, these include operational and financial performance targets.

You will be responsible in ensuring the service is operated in accordance with the CHSP Guidelines, and that quality of client care is delivered.

In this role you will contribute to continuous improvement of the service and ensure that the service aligns to the National Aged Care Standards as well as the National Safety and Quality Health Service Standards. This role will also play an important role in aligning the service with the changes to support at home from July 2025.

Selection Criteria:
1. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia.
2. Recognised post graduate tertiary qualification in management (or willingness to undertake same) and previous experience in management/leadership roles.
3. Demonstrated operational, financial, and human resource experience at a middle managerial level.
4. Demonstrated comprehensive understanding of clinical governance, risk and patient safety when delivering care in the community.
5. Demonstrated experience in quality improvement, clinical policy and procedure development and implementation, as well as experience with the Aged Care Quality Standards.
6. Demonstrated knowledge and skills in managing multidisciplinary teams.
7. Demonstrated high level interpersonal, verbal and written communication skills.
8. Current valid and unrestricted NSW Driver’s Licence
