A staff Speech/Language Pathologist is required to be flexible with daily scheduling and may be required to work various patient care settings. A staff Speech/Language Pathologist is supervised by the Home Care Therapy Manager. Following appropriate practice, the Speech/Language Pathologist evaluates, plans, and implements individual patient care, educating, counseling patients, documents progress, and participates in planning and discharging according to the department policies and procedures. This position may also perform other assigned duties. A staff Speech/Language Pathologist must demonstrate the knowledge and skills necessary to provide age, goal, and diagnosis appropriate care. The individual must demonstrate knowledge of the principles of growth and development throughout the life span, be able to assess patients' needs and status by analyzing evaluated data, and be able to interpret information to identify age-specific needs and provide appropriate care accordingly per patient care setting. This position must also be able to follow and adhere to the practices described in the Department Policy and Procedure Manual. Participates in facility meetings, departmental meetings, in-services, educational programs, and other meetings as required to include solutions and engagement with resolution processes. Speech/Language pathology duties include: initial and ongoing assessments to identify the client's level of functioning and ability to communicate, including OASIS assessments at appropriate time points; developing the plan of care in consultation with the physician and other interdisciplinary care team members and revising it as appropriate; preparing clinical and progress notes; consultation and coordination of services with other interdisciplinary team members; evaluating the effectiveness and outcomes of care; planning for patient discharge; and participation in in services and case conferences. Demonstrates consistent fiscal accountability, by providing recommendations and participates in supply utilization changes that support cost containment. Team member participates and provides feedback in new equipment/product evaluations; identifies and recommends solutions for cost savings and waste reduction.

Qualifications
Education
- Masters Degree, Speech Language Pathology (Required)
Experience
- 1-3 years Recent relevant experience (Preferred)
Licenses and Certifications
- Speech Language Pathologist (SLP) - The American Speech-Language-Hearing Association (ASHA) (Required)
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Required)
- Driver's License (Required)
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) (Required)
