Job Description

Monday-Friday, 8hrs onsite

As an integral part of the Chicago Institute for Fetal Health (CIFH), the Nursing Outreach Navigator develops strong, collaborative working relationships with providers and patients within the Northwestern Medicine and Prentice Women's and Maternal Fetal Medicine Specialties. The Navigator ensures families have seamless, timely, coordinated access to fetal health services with CIFH. The position reports to the CIFH Nursing Director.

Essential Job Functions:
1.    Develops and manages relations with the MFM and OB physician practices and facilitates inquiries and referrals to CIFH.
2.    Develops intimate knowledge of referral dynamics that serves to advise market strategy.
3.    Provides consultation and education to referring providers, within scope of practice.
4.    Develops expertise in care processes within CIFH in order to facilitate seamless referral patterns.
5.    Delivers compassionate, patient centered nursing care coordination at the system level.

6. Communicates and collaborates with other departments and institutions regarding CIFH patient care coordination across the continuum.

7. Assists patients in interpreting care recommendations, in collaboration with CIFH nursing team.

8. Effectively partners and collaborates with internal and external liaisons and resources.

9. Coordinates and participates in conferences and seminars to further the knowledge relating to fetal diagnoses and treatment.

10. Participates in research efforts and quality improvement initiatives.

11.  Prepares quality, volume, and customer satisfaction reports, in collaboration with CIFH Nursing Director.
12. Assists CIFH Fetal Nurse Coordinators with workload and duties, as needed.
13.    Position requires occasional travel within referral service areas.

Knowledge, Skills and Abilities: 
1.    A BSN from an accredited school of nursing and current licensure as a registered professional nurse in the State of Illinois required.  Master’s prepared from an accredited nursing program preferred
2.    Three years of nursing experience in one or more relevant fields required, five years preferred: Maternal Fetal Medicine, Labor and Delivery, Fetal Care Center or Obstetrical nursing
3.    Experience working in partnership with health care professionals and support staff in multidisciplinary setting required
4.    Strong organizational, problem-solving skills required
5.    Proficiency with Microsoft Office and standard office equipment required
6.    Electronic medical record experience required. EPIC experience preferred
7.    Effective written and verbal communication and customer service with patients, families, physicians, and staff required
8.    Basic Life Support certification required
9.    Prior presentation experience required
10.    Strong knowledge of market dynamics in the Midwest region preferred
11.    Prior process improvement experience preferred

Pay Range

$89,440.00-$147,576.00 Salary
