Job Description

RN Care Coordinator – Acute Care Surgical Services (Adult Outpatient Pavilion)

The RN Care Coordinator will be an integral part of the multidisciplinary team at the Acute Care Surgical Services (ACSS) Adult Outpatient Pavilion. This position is focused on ensuring that patients progress through their acute episode of care and post-discharge recovery in a manner that is quality-driven, cost-effective, and safe.

The RN Care Coordinator will not provide direct patient care (such as taking vitals or administering medications) but will be primarily responsible for managing patient care, coordinating services, and optimizing care delivery in collaboration with physicians, social workers, discharge planning assistants, and payers.

Key Responsibilities:

Triage:

o Manage patient phone calls and EMR inbox messages.
o Handle new patient referrals and ensure appropriate routing.

Scheduling:

o Assist inpatient teams with scheduling follow-up visits tailored to patient-specific needs.
o Serve as a clinical resource to the access team for proper patient scheduling.
o Conduct pre-visit phone calls for clinically complex patients to prepare them for upcoming visits.

Chart Prep:

o Review upcoming clinic schedules to confirm patients are correctly booked.
o Gather necessary outside documentation and upload it into patient charts prior to visits.
• Assist with Provider Documentation:
o Prepare documentation for provider visits to ensure thorough record-keeping.
o Place pending orders as appropriate to streamline care delivery.
• Pre-Op and Post-Op Support:
o Provide pre-operative teaching for common surgeries such as ostomy takedown and hernia repair.
o Order pre-op labs and assist with outpatient clearance.
o Provide post-operative telephonic support and follow up with patients on results, studies, and consults.
• Coordination of Outpatient Services:
o Organize home health services and Durable Medical Equipment (DME) supplies as needed.
• FMLA, Disability, and Return to Work Support:
o Take responsibility for handling patient FMLA, disability, and return-to-work paperwork.

• Baccalaureate Degree in Nursing from an accredited School of Nursing
• Active Registered Nurse (RN) licensure in the state.
• Minimum of 3 years of experience in an acute care or outpatient setting.
• Strong organizational and time-management skills.
• Ability to work collaboratively with a multidisciplinary team.
• Knowledge of medical terminology, outpatient care processes, and patient documentation.
• Excellent communication skills, both verbal and written.

Desired Skills:

• Experience in surgical or post-surgical patient care.
• Familiarity with Electronic Medical Records (EMR) systems.
• Ability to provide compassionate care and support to patients and families.

Physical Requirements:

• The position may require occasional lifting, standing, or walking during patient interactions.

Work Environment:

• The RN Care Coordinator will work within an outpatient surgical setting, with regular interaction with patients, providers, and other healthcare professionals. Some tasks may be completed remotely (e.g., chart prep, documentation support).

This role is vital in improving operational efficiency and patient outcomes in the ACSS outpatient clinics while providing high-quality, patient-centered care.

Licensure, Certification, or Registration Requirements for Hire:
Licensed Registered Nurse in the State of Virginia or eligible

Licensure, Certification, or Registration Requirements for continued employment:
Current RN licensure in Virginia Complete 15 Continuing Education Units per year

Experience REQUIRED: Minimum of three (3) years of nursing experience in an acute care setting

Experience PREFERRED:
Academic healthcare experience One (1) year of UM/Care Coordination experience Clinical experience with specialty patient population

Education/training REQUIRED:
Baccalaureate Degree in Nursing from an accredited School of Nursing,or a Bachelor’s degree in a biological, health or social sciences field from an accredited school.

Education/training PREFERRED:
Master's Degree in Nursing or a healthcare related field from an accredited program

Independent action(s) required:
Works with the attending and consulting physicians to facilitate effective and efficient transition through the process of hospitalization.
Works collaboratively with all members of the multi-disciplinary team to ensure patient needs are met and care delivery is coordinated across the continuum, as well as appropriately reimbursed by payers as contracted, and that resources are efficiently and effectively utilized.
Seeks the expertise of social workers to resolve psychosocial patient care issues and to develop complex patient transition/discharge plans as needed. Interacts with patients, family members, healthcare professionals, community and state agencies in this effort. Serves as an advanced clinical resource to the team.
Serves as a liaison between the hospital and community agencies or facilities for the exchange of clinical and referral information.
The role is responsible for maintaining hospital compliance with the Quality Improvement Organization (QIO) series and CMS guidelines.
Utilizes medical necessity criteria as a tool to assess appropriateness of level and setting of care, assists in the denial and appeals process, assesses quality and identifies and reports potential risk management issues.
Responsible for participation in and completion of all patient safety initiatives appropriate to the position.
The incumbent conducts all job responsibilities according to VCUHS’ Mission and Values.
Responsible for establishing and maintaining a professional rapport with providers, patients/families, and internal customers.
