Basic Purpose:

The RN - MDS (Minimum Data Set) Coordinator is accountable for coordinating and overseeing the full collaborative, interdisciplinary assessment and care planning process in accordance with all federal, state, and agency policies, procedures and regulations.  The goal of the assessment and care planning process is to promote the resident’s quality of care and life, including individuality, safety, wellness, satisfaction and dignity.

Typical functions:

The functions performed by employees in this position will vary but may include the following:

  • Monitor the 24 Indicators for staff documentation and trending.  Provide bi-annual CQI reports on the 24 quality indicators.

  • Comply with Pro Care Tracking guidelines by assigning residents to IDCP within designated timeframes, locking/unlocking and transmitting MDS.

  • Ensure new MDS/care plans are initiated on residents with significant status changes within the time frames required by VA guidelines and policies and procedures.

  • Maintain an annual calendar for names of current residents scheduled for weekly IDCP meeting and update list with newly admitted and discharged residents.  Schedule new residents plan using established time frames.

  • Review “Fall” reports daily for specificity and appropriateness of intervention, identifying doctor’s order for all restraints.

  • Notify each house supervisor on the status of each nurse’s MDS/care plan and provide a copy to the Assistant and Director of Nursing plus the Assistant Administrator and Administrator.

  • Provide weekly schedule of residents scheduled for IDCP to Treatment Team members.  Conduct IDCP meetings maintaining resident-centered focus.  Provide Pro Care updates and guidance to the team members in order to meet and maintain VA standards.  Teach MDS and Care plan process during RN new hire orientation.

  • Maintain current nursing licensure with the OK Board of Nursing as a Registered Nurse.  Maintain C.P.R. certification and proficiency in both technical skills and interpersonal skills to promote resident centered, intershift communication.

Knowledge, Skills and Abilities required at this level include knowledge of available health programs and resources; professional nursing theory, practices and techniques; health promotion and disease prevention; community health programs and resources; nutrition and diet therapy; the functions of various clinical departments; the principles and practices of supervision; nursing administration procedures, personnel principles and practices; federal, state, and agency rules and regulations concerning health services; accreditation, certification and licensure standards; principles and methods of teaching and nursing standards.  Ability is required to plan, organize and apply nursing care techniques; counsel and teach patients and families; instruct and monitor the work of others; develop, plan, and implement an effective nursing care program and to provide education and training.   Strong assessment and analytical skills, attention to details, a thorough understanding of MDS requirements and a commitment to person-directed care is essential.

Education and Experience requirements at this level consist of possession of a valid permanent Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing and one year of professional nursing experience plus an additional year of professional nursing experience in a specialized field of nursing appropriate to the health care setting such as public health or psychiatric nursing or the completion of the requirements for a bachelor’s degree in nursing may substitute for the required one year of professional experience but one additional year of professional nursing experience in a specialized field of nursing appropriate to the health care setting such as public health or psychiatric nursing is required.
