We're looking for an enthusiastic, skilled Physical Therapist with a Doctorate in Physical Therapy to join our team and work with a vibrant group of healthcare professionals. This role offers the opportunity to evaluate and treat patients within 24 hours of referral, ensuring quick and personalized care. You’ll be at the forefront of developing effective treatment plans, collaborating closely with physicians to ensure the best outcomes for every patient!
In this exciting role, you’ll:
- Evaluate patients quickly and efficiently within 24 hours of referral to ensure timely care and personalized treatment plans. If there’s ever a delay, you’ll take charge and document the reasons in the medical record to ensure transparency and accountability.
- Create and implement personalized treatment plans, ensuring alignment with physician instructions, while also empowering patients to make progress toward their goals.
- Supervise and guide Physical Therapy Assistants, aides, and students in their patient care and therapy-related activities, helping them develop professionally while adhering to the state practice act.
- Collaborate with a team of experts—supervisors, nurses, and other healthcare professionals—on patient progress, goals, and care plans. You'll play a key role in shaping the direction of patient care through active participation in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences.
- Participate in educational programs to train and support other staff in the facility through in-service training, sharing your knowledge and passion for physical therapy.
- Maintain detailed documentation of evaluations, treatment notes, progress reports, and certifications, ensuring compliance with state practice acts and payer requirements.
- Instruct families and caregivers in the maintenance programs and provide valuable training to ensure that patients continue to thrive after discharge. - Assist with discharge planning, ensuring a smooth transition for patients leaving therapy and empowering them with the tools they need for continued success.
- Help maintain department equipment, ensuring everything stays in top working condition for patient care and safety.
Who You Are:
- A licensed Physical Therapist with a passion for helping others.
- Equipped with a Doctorate in Physical Therapy and a commitment to continuing education to stay current with best practices.
- A proficient computer user, familiar with programs such as Casamba and Point Click Care to streamline patient care and documentation.
- A strong communicator who can clearly explain treatment plans and collaborate effectively with patients, families, and the healthcare team.
- Able to supervise and lead Physical Therapy Assistants, aides, and students, helping them grow in their careers while delivering excellent care.
- Someone who thrives in a flexible, team-oriented environment where you can make a real impact in patient care.
Physical Demands:
- You’ll need to be on your feet frequently, helping patients through their rehabilitation with hands-on care, lifting up to 100 lbs as needed.
- You’ll be active and moving, reaching, pushing, pulling, and ensuring that every patient’s needs are met.

Summary of Benefits
