Job Summary

This unique role utilizes skills in a mostly non-clinical manner. The RN provides home visitation and case management for families referred from Denver Human Services. Under minimal supervision this position facilitates, coordinates and directs medical care, resources and education for families. The RN participates in appropriate team decision making meetings and other interdisciplinary care rounds to review treatment goals, progress and identified needs. The nurse is responsible for a caseload of 24 families.

Essential Functions:

  • Facilitates effective and appropriate use of healthcare resources with clients. (40%)
  • Identify community resources which can aid with needed health supplies and equipment. (25%)
  • Participation in interdisciplinary meetings. (20%)
  • Collaborates with other members of the healthcare team and specialists as needed. (15%)


Education:

  • Bachelor's Degree Bachelor's of Science in Nursing Required


Work Experience:

  • 1-3 years RN experience Required


Licenses:

  • BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association Required and
  • RN-Registered Nurse - DORA - Department of Regulatory Agencies Required


Knowledge, Skills and Abilities:

  • Strong oral and written communication skills
  • Ability to demonstrate an understanding of assessment findings as they relate to the disease process and continuum of care.
