Job Description
Under the supervision of the Care Manager-RN and/or Licensed Social Worker, the Care Manger assists with patient flow/throughput in collaboration with the patient/family, physicians, interdisciplinary team, and payer representative. The Care Manager ensures patient progression through the continuum of care in an efficient and cost-effective manner, to assist with placing the patient in the appropriate care setting. Identifies, initiates and manages optimal care coordination and discharge planning to develop patient flow/throughput to enhance continuity of care, smooth and safe transitions, patient satisfaction, patient safety and length of stay management.Education Qualifications
- Bachelor's Degree in social work, psychology, or like area of interest is required.
- A licensed practical nurse (LPN) or other health care license and 5 years of health care experience may be considered in lieu of a bachelor's degree.
- Prior experience in social work or past clinical social service experience, directly transferrable to a healthcare environment, is preferred.
- Basic Life Support Certification (BLS) is required within 60 days of hire.

Pay Range:
$24.31 - $36.47A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education.
