The Complex Care Nurse Navigator (CCNN) reports directly to the Director of Case Management who works in close collaboration with the ACNO of Nursing Operations. CCNN serves as a point of contact for patients identified with high-risk diagnoses such as (but not limited to) Congestive Heart Failure, Stroke, MI, COPD. CCNN coordinates with multidisciplinary teams across the continuum and is involved in activities associated with these patients to ensure optimal transitions of care. CCNN will assist in data collection and performance improvement in collaboration with the hospital's Quality department. CCNN will work as part of a multidisciplinary team to provide education and guidance to our complex care patients. This clinical provider will provide patient care within regulations in accordance to their licensure within the Commonwealth of MA.
Responsibilities:
CCNN will perform daily rounds on inpatients with congestive heart failure, stroke, MI and COPD as primary diagnosis as well as other complex care patients identified.
Supports the stroke program and maintains regulatory compliance with state and national standards for designated stroke centers. In collaboration with the Stroke Program Manager.
Collaborates with medical and hospital staff to provide multidisciplinary care to the patients identified.
Collaborate with stakeholders to develop the necessary educational components and teaching material for staff, patients, and the community
Supports the development of performance improvement teams and/or coordinates with leadership
Ensures the protocols and therapies become the standard of care followed within the facility
Act as a member of the Stroke Committee and Readmission committee providing guidance on processes and workflows. Serve as a leader in patient education, working with bedside nurses, therapists (PT, OT, SLP), and the provider team to provide education to patients and/or families
In conjunction with Case Management and social work, will assess caregiver readiness
Provides education and counsel patients and their family/support network on care and services
Conduct follow-up phone calls to improve transitions of care, patient safety, and patient satisfaction
Collaborate with Community Outreach Director and community agencies to provide community outreach and education initiatives and support groups
Job Description
Minimum Qualifications:
1. Massachusetts RN Licensure.
2. 3 Years of RN Experience.
3. Current Basic Life Support (BLS) Certification.
Preferred Qualifications:
1. Bachelor of Science in Nursing (BSN).
Hours: 40 hours/week, Day Shift
