Salary: $72,152.53 - $101,229.48 base salary per annum

About the role 

  • Case Manager as part of the Community Mental Health Team

  • Lead, direct and co-ordinate crisis and risk assessment.

  • Collaboration with internal/external providers including referrals to other services.

  • Undertaking duties relevant to the NSW Mental Health Act 2007.

  • Current unrestricted NSW Drivers Licence

About you 

  • Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.

  • Demonstrated experience working within a Community Mental Health Position as part of a multidisciplinary health care team.

  • Demonstrated experience working with people on Community Treatment Orders is desirable.

  • Demonstrated ability to deliver high quality consumer care and services to support families and carers of people with mental illness.

  • Commitment to provision of recovery-focused holistic mental health interventions.

In this role you will be based at St Vincent’s hospital, Darlinghurst.  

  • A busy inner-city hospital in a beautiful location

  • Closely partnered with universities & affiliated research institutes, many of which are located on-campus 

  • Centres of excellence include heart & lung transplantation, bone marrow transplantation, cardiology, cancer care, AIDS/HIV, respiratory medicine, mental health, homeless health, drug & alcohol services, and correctional health
