- Shift Details: 6:45 am to 7:10 pm
- In this role you will play an integral part in a patients care by helping to facilitate internal and external transfers
- Saint Luke's Hospital of Kansas City is Magnet Designated and a Level 1 Trauma Center
Self-directed and functions independently to facilitate internal and external patient access to SLHS. Demonstrates increasing discriminatory judgment in interpreting data and prioritizes appropriately. Utilizes the nursing process, requiring ability to work independently, with strong problem solving skills, critical thinking. Work effectively and cooperatively with diverse members of the health care team. Communicates clinical issues and address these issues immediately via proper System channels. Serves as resource to colleagues at the unit or department levels.
Facilitate external and internal SLHS transfers
Collaborate with Transfer Physicians, Referring Providers and other healthcare professionals as needed to facilitate a safe and appropriate transfer
Serves as a tele-health nurse while completing discharge follow up calls with identified patient populations.
