Are you interested in working for a company that makes a difference?  If so, Keystone Human Services has an exciting opportunity for a Health Services Manager or Community-Based Nurse Manager.  This hybrid position is responsible for overseeing nursing services for both Mental Health and Autism Services.

The Health Services Manager or Community-Based Nurse Manager is responsible for applying healthcare knowledge and leadership to Keystone's Autism and Mental Health Divisions so teams can effectively support individuals served with their health care needs. This is a hybrid position, working from home and within our community-based locations. This position will work collaboratively with people at various levels of the organization and outside organizations to address the medical needs of the individuals in adherence with Keystone’s mission and in compliance with all regulatory requirements.
 

The Work:

• Supervise a team of LPNs across multiple locations/counties.
• Evaluate, monitor, and coordinate health care functions to assure consistency and regulatory compliance in service delivery. 
• Develop, prepare, and deliver educational information and training to enhance the medical knowledge of all employees. 
• Consult with LPN’s and Directors concerning the health status of individuals supported and to ensure there is adequate follow up regarding medications, appointments, and provide recommendations where needed. 
• Assist with the assessment of referrals to evaluate appropriateness of placement; participate in pre-admission, admission and discharge/transfer of individuals as related to health care needs. 
• Routinely complete on-site visits to ensure quality and efficiency of services. 
• Assist with licensing and audits related to health care services.

Minimum Requirements:

• Must possess current valid RN licensure.
• At least three (3) years of work experience in the medical field
• Experience working with individuals living with autism and/or mental illness strongly preferred.
• Supervisory experience is required. 
• Ability to apply proficient computer skills, including the use of software and web based systems.
• Must demonstrate effective written and verbal communication skills; ability to read, interpret and apply policies and regulations.
• Able to pass all required background checks and clearances.
• Must possess a valid driver's license and have daily access to a properly insured vehicle.
