Job Description

This position reports to the Director of Home Health and Specialty Pharmacy leadership. This position is responsible for the home infusion patient’s transition from hospital to home, and in home education and infusion administration assistance if needed. You will collaborate  to facilitate an effective transition of care, and have ownership of home infusion education and will work with patients in their home and our main metro locations to ensure successful home infusion therapy outcomes. This position provides care to adult patients with variety of conditions and therapies. The Home Infusion Nurse will work towards teaching the patient to manage home infusion therapy on their own.  Strong time management, self-scheduling, and peripheral IV skills are required for this position. We are looking for someone who is flexible, adaptable, patient focused, confident in their IV and nursing skills and comfortable working autonomously on a small team. Is responsible for coordinating the care for a group of patients and consulting with other members of the health care team when indicated. Provides nursing care in compliance with the Nurse Practice Act.

Responsibilities Include:

• Build relationships with patients and peers
• Educate patients on home infusion processes for medication administration, line care, line maintenance and starting infusions on their own
• Coordinate patient education with nursing and care coordinator staff
• Peripheral IV line placement
• Assist with starting infusions in the patient’s home
• Assist with medication administration via ambulatory pump, gravity, or push in the patient’s home

• Assist with line care and maintenance in the patient’s home
• Work with peers to assist in transitional infusion care.
• Create and update patient education materials
• Traveling throughout the greater Kansas City Metro to assist patients with their infusion needs at our metro locations and in the patient’s homes.

Preferred: Experience with TPN and enteral nutrition services
