Adventist HealthCare seeks to hire an experienced Transitional Care RN for our Care Navigation department who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing.
As a Transitional Care RN, you will:
• Identify, assess, plan, implement, coordinate, monitor, and evaluate healthcare services to meet an individual patient's specific healthcare needs cost-effectively (may include but not limited to home health assessment, telehealth initiation, disease management education, coordinating of post-acute services)
• In collaboration with the Community Health Worker, coordinate patient care with patient, family, staff, physicians, and ancillary/community services to promote quality and cost-efficient care for assigned caseload
• Ensure weekly contact with the patient (in person or via telephone) to progress toward care plan goals
• In collaboration with the Community Health Worker, act as an advocate for the connection with community resources.
• Conduct patient home assessment visit when indicated
Qualifications include:
• Graduate of an accredited school of nursing with a minimum of an associate of science degree required, bachelor’s degree preferred
• 1-3 years of experience in an acute care setting, and at least 1 year of prior experience as a case manager, discharge planner, or care coordinator desired
• Maryland RN licensure
• Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance
• Active American Heart Association Basic Life Support (BLS) certification required

Pay Range:
$71,932.12 - $107,889.60If the salary range is listed as $0 or if the position is Per Diem (with a fixed rate), salary discussions will take place during the screening process.
