The Wound Care Registered Nurse (RN) primarily focuses on assessing documentation from field staff and collaborating closely with physicians to manage patients with a variety of complex wounds and skin care needs.

The Wound Care Registered Nurse (RN) plays a vital role in creating personalized treatment plans that align with medical prescriptions and best practices in wound care while also focusing on utilization control to enhance patient outcomes. The expertise of a Home Health Wound Care RN is essential for effectively managing a wide range of conditions, from acute injuries to chronic ulcers, ensuring that each patient receives optimal care tailored to their unique needs.

Key Responsibilities:

  • Develop and implement evidence-based wound care protocols and standardized best practices to enhance patient outcomes.
  • Assess and provide consultation for both chronic and acute wounds, including ulcers, burns, pressure injuries, diabetic foot wounds, and skin tears.
  • Introduce strategies aimed at accelerating wound healing and preventing infections.
  • Educate home health clinicians and physicians on effective wound management techniques.
  • Specialize in the care of patients with ostomies, such as colostomies and ileostomies, ensuring they receive comprehensive support and education.
  • Train and assist clinicians in ostomy care while managing related complications.
  • Guarantee that patients receive appropriate ostomy care in their home environments.
  • Address issues related to urinary and fecal incontinence, providing strategies and support to enhance patients' lifestyles and overall quality of life.
  • Collaborate closely with the Director of Nursing, Case Managers, and interdisciplinary teams, including LPNs, NPs, physicians, dieticians, and therapists, to ensure coordinated care.
  • Work with post-acute care providers to maintain cohesive and evidence-based treatment plans.
  • Oversee and optimize utilization management processes within the organization, ensuring efficient resource use that improves patient outcomes while managing costs.
  • Adhere to the wound care formulary, recommending treatments that align with agency guidelines.
  • Conduct prospective, concurrent, and retrospective reviews to assess treatment efficacy and compliance with evidence-based practices.
  • Analyze wound healing rates and evaluate the effectiveness of specific wound care protocols to continuously improve care delivery.

Required Qualifications:

  • Possess compact Florida and Texas RN license.
  • Certified Wound and Ostomy Care Nurse (CWOCN or WOCN).
  • Demonstrated expertise in wound care, ostomy care, and continence management.

  • Proficient in OASIS documentation.
  • Strong knowledge of best practices in wound management and treatment.
  • Exceptional interpersonal and educational skills, enabling effective teaching and mentoring of clinical staff.
  • Excellent collaboration abilities to work seamlessly within diverse healthcare teams.
  • Detail-oriented with a strong emphasis on accuracy in patient documentation.
  • Capable of working independently as well as collaboratively within a team.

Preferred Qualifications:

  • Proficient in WellSky/Kinnser software
  • Bilingual, fluent in both English and Spanish

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


 

$77,200 - $106,200 per year
