Are you a dedicated and compassionate Registered Nurse (RN) with a passion for pediatric care and care coordination? We are seeking a Pediatric RN Care Manager to join our team and play a key role in coordinating patient care, supporting families, and ensuring seamless transitions across healthcare services.

Job Summary
Key Responsibilities:•Serve as a core member of the care team, coordinating healthcare services within the facility and referrals to external providers.
•Conduct patient assessments and collaborate with families to develop personalized care plans.
•Provide education and support to patients and families to help them navigate healthcare options.

•Partner with service providers, families, and patients to ensure continuity of care and access to community resources.
•Maintain detailed reports and records to track patient progress and treatment plans.
•Participate in case reviews with medical directors to discuss challenging cases and determine best treatment approaches.
Qualifications
- Registered Nurse (RN) license in Massachusetts
- Basic Life Support (BLS) certification preferred.
Additional Job Details (if applicable)
- 2+ years of inpatient/acute care nursing experience OR 2+ years of care management experience.
- Pediatric nursing experience.
