POSITION PURPOSE:

The Nurse Health Navigator will serve on the front lines of one of our most integral programs, helping adults with serious an persistent mental illnesses/emotional disorders. – Will serves as a critical liaison between healthcare providers, patients, and community resources. This role focuses on improving patient outcomes by guiding individuals through the healthcare system, addressing social determinants of health, and connecting them with community-based support services.

WHAT YOU WILL DO:

  • Assess persons served needs and develop individualized care plans.
  • Provide education on chronic disease management, preventive care, and available community resources.
  • Collaborate with physicians, social workers, and other healthcare professionals to coordinate care and ensure smooth transitions between healthcare settings.
  • Assist persons served in scheduling appointments, understanding treatment plans, and overcoming barriers to care.
  • Serve as the primary departmental liaison with CCBHO and the Service Coordination Unit for the Wellness initiative.
  • Provides education to the Service Coordination unit and to clients on identified Wellness metrics and other relevant topics as needed.
  • Track clients who meet the metrics set by CCBHO for Wellness, provide required documentation of those metrics to both the assigned Service Coordinator and CCBHO.
  • Exercise learned medical and clinical judgment in initiating services, scheduling appointments, developing natural and community support systems, and insuring appropriate educational goals are met for clients of the Service Coordination Unit in collaboration with the assigned Service Coordinator.

MINIMUM QUALIFICATIONS:

  • BSN degree and 1 year of nursing experience required OR Associate RN Degree and 2 years nursing experience required
  • Psychiatric, community-based nursing experience strongly preferred  
  • Current valid RN license in the State of Pennsylvania.
  • Act 33/34 and FBI clearances.
  • Valid driver’s license.
  • Ability to drive an organization vehicle
  • Pre-employment drug screen
  • Certified Drug and Alcohol Counselor certification is desired

HIGHLIGHTS/BENEFITS

WORK SCHEDULE This is an Exempt (salary) position.

Monday-Friday, 9:00 AM - 5:00 PM, with flexibility to meet consumer needs.  Some non-traditional hours (evenings, weekends and holidays).
