Performs as an operational extension of the Children's health management service to provide case management and care coordination services for target/eligible population(s). Assesses, plans, implements, coordinates and evaluates the plan of care in partnership with the family and other members of extended care team. Subject matter expert and workflow specialist that will support continuity and consistency in workflow for various care coordination activities in various sites/areas across the system. Effectively uses data to evaluate clinical practice and makes recommendations to optimize productivity, care outcomes, safety and patient experience.
Bachelor's Degree in Nursing required.
Three years clinical nursing experience
Knowledge of pediatric chronic condition management and mental/behavioral health experience preferred.
A current license to practice professional nursing in the state of Wisconsin.
Requires a high level of analytical ability to work with complex patients/families medical and social problems across the continuum.
Communication and interpersonal skills which demonstrate effective collaboration with patient, family and health care team members across the continuum.
Demonstrated analytical abilities, judgment, and initiative necessary to work from general instructions in response to complex and sensitive issues and problems.
Advanced skills in use of Word, Excel, Power Point, and EHR (Epic preferred).
Excellent consultative, collaboration and communications skills necessary to partner with teams from Children's Wisconsin, Children's Specialty Group, social and community services programs, physicians, patient/families and others from the community to achieve excellent patient/family and provider experience.
