Under the direction of the practice physician and/or advanced practice provider (APP) and the supervision of the Nurse Manager (RN) and/or Nurse Supervisor (RN), the RN may provide direct patient care, patient triage (in-person and telephonic), assessment, planning, directing and evaluating of a patient’s specific care plan and clinic visit regarding elevated blood Lead levels and care. Work with NYS Counties, Hospital Providers, PCP Providers, Parents, and Department of Health Coordinators to coordinate care both in clinic and outpatient. Report to Syracuse Regional Lead Resource Center as well as provide quarterly and annual reports. Able to work effectively within a team of multi-specialty professionals as well as independently.
Essential Duties and Responsibilities include:
Performs review and triage of incoming test results, patient requests and pharmacy renewals; facilitates call-backs to patients as necessary
Facilitates the utilization of resources to meet patient outcomes and contribute to Facility (AMC), New York State and National Quality data metrics
Collaborates with the medical team to create a Plan of Care for the patients
Delegates tasks deemed appropriate per licensure of LPN and/or MA
Adheres to AMC’s regulatory agency (The Joint Commission) and internal compliance policies
Completes continuous/annual education related to Clinic specialty to maintain knowledge base on current practices
Proactively monitor patients' clinical process through /patient care standards and evidence-based guidelines to ensure timely, appropriate interventions that achieve optimal patient outcomes
Provides education to patients related to their Lead levels and Lead health and wellness
Coordination of care of Lead poisoned children with programs overseeing Provider
Coordination of care of Lead poisoned children between NYS Northeast Counties via NYSCHECK
Coordinate care of Lead poisoned children with The Department of Health
Tracks patients Lead levels, follow-ups and continued care
Demonstrates experience in the referral process and use of community resources
Plans and performs Community Outreach Programs to provide community education
Minimum Qualifications:
Bachelor’s degree of a state accredited Registered Nursing program is preferred
Minimum of 1-3 years of previous clinical RN experience-pediatrics preferred
NYS current RN licensure to practice professional nursing is required
Current CPR certification required
Excellent customer service and communication skills
Ability to effectively present information and respond to questions from physicians, patients and their family members or other employees within the work setting. Familiar with computer software.
Effective Written and Verbal Communication Skills
Working Knowledge of MS Word and Excel
Experience with electronic health record preferred
