About ThedaCare!

Schedule: M-F 8a-4:30p This position requires a weekend rotation of every 8 weeks and 1 holiday per year.

Location: ThedaCare Hospice covers 13 different counties split up into 5 different teams of Case Managers.

KEY ACCOUNTABILITIES:

  • Utilizes the nursing process, evidence-based practice, and specific competencies to assess the physical condition and nursing needs of patients, and develops a plan of care in a collaborative practice with the patient and interprofessional team.

  • Plans for the care needs of the patient in collaboration with the interprofessional team to provide the highest quality of care and clinical outcomes.

  • Demonstrates clinical expertise in the provision of care in the clinical specialty assigned, and performs all functions of the professional clinical nurse (RN), which are age appropriate, developmentally sensitive, and culturally specific.

  • Identifies ways to improve the patient’s experience of care, streamline care processes, and lower costs while promoting quality to improve patient, family, and team member satisfaction.

  • Contributes to a professional environment that encourages mentoring, engagement, and development to retain expert clinicians.

  • Provides consultation and maintains positive relationships with physicians and other interprofessional team members, collaborating to problem solve and improve patient care.

  • Demonstrates, anticipates, and proactively manages risk to prevent crises. 8. Performs skillfully in life threatening emergencies, matching demands and resources during crises situations.

QUALIFICATIONS:

  • Current Wisconsin RN Licensure

  • Bachelor's of Science in Nursing (BSN) or commitment to obtain within five years.   Associate's Degree in Nursing accepted in lieu of BSN for those hired prior to 1/23/17.

All 2500 RN Clinical Nurse:

  • American Heart Association Healthcare Provider Basic Life Support (BLS)

 

PHYSICAL DEMANDS:

  • Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of Fifty (50) pounds without assistance

  • Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties

  • Manual dexterity and hand-eye coordination to perform patient care procedures

WORK ENVIRONMENT:

  • Frequent exposure to sharp objects and instruments

  • Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock

  • Occasional high noise level in work environment

  • Standing and/or walking for extended periods of time

  • Transporting, transferring, positioning patients and/or equipment from one location to another; little likelihood for injury if proper body mechanics and procedures are followed

  • Possible exposure to communicable diseases, hazardous materials, and pharmacological agents

  • Occasional contact with aggressive and or combative patients.

Position requires compliance with department specific competencies.

The ThedaCare Acute Hospice Admission Nurse is a registered nurse and an Interdisciplinary Team Member whose primary responsibility is assist with acute inpatient admissions, this includes reviewing hospice services and philosophy to patients and their caregivers.

This RN will assist in completing all acute admissions, which typically take place in one of the ThedaCare hospitals. This nurse will work directly with the hospital team to ensure patients acute symptoms are managed.

***Knowledge of Acute/GIP hospice a plus!***

Essential Job Functions and Responsibilities:

  • Explain the hospice philosophy of palliative care with the patient and family/caregiver as unit of care.
  • Determine the most appropriate setting for patient to be receiving care.
  • Start discharge planning from acute setting upon admission.
  • Explain the patient’s rights and responsibilities
    • Provide the patient with a copy of ThedaCare Hospice notice of privacy practices.
    • Assess the family/caregiver’s ability to provide care.
    • Evaluate physical facilities and equipment in the patient's home to determine if they are safe and effective for care in the home.
    • Allow the patient and family/caregiver to ask questions and facilitate a decision for hospice services especially provided under the Medicare/Medicaid hospice benefit.
    • Review appropriate forms and subsequently sign forms by patient and family/caregiver once agreement for the hospice program has been decided.
    • Provide services as needed and ordered by physician (or other authorized independent practitioner), and incorporate additional needs into the hospice plan of care.
    • Give patient information about durable power of attorney for health care, if the patient has not already done so.
    • On admission, the admitting registered nurse will discuss the patient’s eligibility for the Medicare or Medicaid hospice benefit.
    • The admitting registered nurse will explain the hospice election statement to the patient and family/caregiver or legal representative.
    • He/she is responsible for seeing process through to completion, including making sure all signatures are obtained and admission is deemed billable.
    • Responsible for starting and maintaining relationships with facilities, Physicians, and hospitals, in an effort to facilitate admissions
    • Assists in QAPI, and chart reviews to ensure compliance in all areas
    • Position also requires that if there is a shortage of nurses due to illness, bad weather, or vacations, the admission nurse would help cover visits.
    • Keeps abreast of all compliance issues and eligibility requirements
    • Performs all other tasks as assigned by supervisor
