Position hours are 8 hours per week - flexible schedule

Overview
Works under the supervision of the Director of Adult Day Services and is responsible for monitoring and documenting health needs of participants for the Center. The Registered Nurse is responsible for completing Medicaid documentation, health assessments and care plans on all assigned Participants.

What you will do

• Develops and updates Medicaid care plans, Medicaid monthly assessments and DSS care plans and health assessments in compliance with DSS and DMAS regulations.
• Maintains Medicaid participants' records and documents progress.

• Performs monthly health assessments. Monitors participant health needs, documents changes and contacts authorized person(s). Provides on-site nursing care as needed.
• Meets with staff to review quarterly care plans and implement changes that need to be made to meet the updated care plan.

Qualifications

Education

• Registered Nurse (Required)

Experience

• Acute Care, Home Health or Long-term Care (Preferred)

Licenses and Certifications

• Registered Nurse (RN) - State Department of Health Professions (Required)
• CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Required)
