JOB RESPONSIBILITIES

HEARING SCREENING
Performs newborn hearing screens on the Mother Baby Unit on all newborns, NICU and newborn nursery when eligible, and Pediatrics when applicable/ordered. Is able to interpret results accurately and reports results to parent(s). Answers questions that pertain to the screening method, equipment used, and results of the screen. Has a working understanding of the Hearing Screening Program and is able to answer basic questions regarding the program. Distributes information as requested. Works with Central Distribution and Central Valley Business Forms to ensure the proper inventory level on all newborn hearing screening supplies and brochures at all times. Responsible for all data collection, data management, and data management software program. Keeps daily records and communicates logs with the California State Hearing Screening Department. Sends monthly reports to the State as required. Responsible for notifying parents and primary care providers of hearing screening results. Documents results in the patient's record and on the legal birth certificate. Follow up outpatient appointments are made for patients who missed their test prior to discharge or who refer (initially fail) according to the Hearing Screening Policy. Responsible for notifying the parents and primary care practitioner if hearing screen is missed and schedules a follow up appointment for outpatient screening. Communicates with referring or receiving hospitals to ensure all neonates are tested. Responsible for scheduling appointments with an Outpatient Hearing Screening Provider as applicable. Ensures that the parent receives the proper paper work if a hearing loss is suspected or a diagnostic evaluation is indicated, the primary provider is notified and the social worker is contacted. Responsible for correctly applying charge for hearing screening service to correct patients account. Responsible for cleaning and maintaining the machine in proper working order. Works with Clinical Engineering to ensure proper/updated calibration certifications are kept. Responsible for sending statistics and certifications to the State Hearing Department each year. Is up to date with annual hearing screen competencies set forth by the State, above and beyond mandated hospital competencies. Communicates with the current State Hearing Screen physician for recommendations and referrals. Links information to the medical record, patient's family, and primary physician. Is responsible for complying with all requests from the State on an annual and as needed basis. Pulls medical records, screening logs, calibration certificates, competencies, and any other documents requested by the State.
BIRTH CERTIFICATE
Makes patient packets which include birth certificate information worksheet, social security number request form and social security release of information. Obtains information from the patients chart and the patient themselves. Enters the information into the AVSS (state program) and verifies this information. Obtains Declaration of Paternity paperwork for unmarried patients wishing to add another parent to the birth certificate and completes this process by sending them to the appropriate county agencies. Completes the birth certificate process by obtaining parent(s) signature(s). Prepares the completed birth certificates for pick up by Tulare County representatives. Completes amendments to the birth certificates as necessary. Checks refrigerator temperatures on Pediatrics, Labor and Delivery and Mother/Baby. Assists patients, local and state governmental agencies with any questions, concerns or changes they may have on a case by case basis. Performs other secretarial duties as needed.

Able to effectively communicate and care for patient and family as reflected in the Plan for Provision of Care.

Pay Range
