JOB DUTIES AND RESPONSIBILITIES:
Monitor clinical dashboards and respond promptly to alerts, ensuring timely resolution to patient concerns
Utilize a strong clinical foundation to effectively triage and assess patients' needs by applying nursing best practices
Provide accurate and relevant information using protocols and other evidence-based tools while maintaining sensitive and confidential information
Employ a patient-centered approach that provides a superior patient experience
Identify quality of care issues and escalate appropriately
Utilize excellent communication skills to communicate in a clear, concise, and timely manner
Demonstrate the ability to work autonomously and collaboratively with other team members, including the patient’s care team
Deliver general health information, education, and care guidance through comprehensive nursing assessments
Facilitate follow-up appointments as necessary for patients discharged from an acute care setting
PHYSICAL AND SENSORY REQUIREMENTS:
Requires sitting up to eight hours per day. Routinely use upper extremities; occasionally to lift items up to 10 pounds. Stoop, bend, and reach above shoulder level regularly. Hearing is required, being sharp and alert. General, near and peripheral visions are all required.
EDUCATION:
Current RN Licensure in the state of practice. Bachelor’s degree in nursing (BSN) preferred from an accredited institution.
TRAINING AND EXPERIENCE:
Three to Five years of clinical experience required. Seven years of clinical experience in direct patient care preferred. Experience with patient education and/or care coordination preferred. Spanish or second language preferred. Exceptional verbal and written communications skills. Strong ability for conceptual thinking, problem resolution and planning. Intermediate knowledge and skill of computer programs such as Microsoft Word and Excel.
