JOB SUMMARY:
The Registered Nurse (RN) Supervisor of Palliative Care Quality, Education and Development supports the AVP & Director in oversight and designing, developing, coordinating, implementing, & management of a comprehensive Palliative Care program. Overall departmental objectives are to address organizational needs across the continuum of care within the St. Tammany Health System. Specific duties include developing and maintaining quality improvement programs, collaborating with the medical staff, interdisciplinary teams, and contracted vendors, as well as community resources to ensure effective delivery of Palliative Care Services. Duties also include serving as a resource to the Palliative Care team which includes collaboration, education and development with other hospitals, outpatient departments, physician clinics, nursing and assisted living facilities. Responsible for overseeing the monitoring, billing processes, and creating reports to show activity, successes, and opportunities for department. Other duties include preparation of statistical reports, marketing, and education of Palliative Care services to staff, physicians and community.
MINIMUM QUALIFICATIONS:
- Graduate of an accredited School of Nursing or a Licensed Healthcare Professional required. Bachelors in Nursing or Healthcare related field is JOB IS FROM: nursingjobs.siteVIEWpreferred.
- Current R.N. licensure with Louisiana State Board of Nursing with no restrictions.
- 3-5 years’ experience in outpatient and/or acute care is required.
- 3-5 years’ experience in Transitional Care, Palliative Care, Home Health or Hospice is preferred.
- Current Basic Life Support (BLS) certification through any International Liaison Committee on Resuscitation (ILCOR) provided courses such as but not limited to: Ochsner Health System, American Heart Association, and American Red Cross. The ILCOR course must include a manikin check off. ACLS and/or PALS, is preferred.
- Extensive knowledge of the healthcare system infrastructure, RPM services, federal and state regulations, as well as reimbursement is required.
- Previous experience in leadership is preferred, with the ability to quantify past management achievements.
- Experience in directing and coordinating programs, projects, resources across multiple functions and geographic locations preferred.
- Expands relationship building to include team members in STHS’s system of care, STHS partners and impacted communities.
- Demonstrates the ability to develop and grow a new service line.
- Identifies indicators to track that support strategic initiatives.
- Acts as a resource to develop, assist, and educate physicians, physician advisors, and interdisciplinary team members and patients.
- Monitors monthly reports, develops and implements action plans to address opportunities for improvement.
- Targets process improvement opportunities that positively contribute to the hospital’s quality and strategic initiatives.
- Maintain quality metrics for Palliative programs and charge reconciliation.
- Must have a valid and current state issued driver’s license. Must have reliable transportation for work with current safety inspection (brake tag) and vehicle license plate registration. Must maintain and provide proof of automobile insurance (at his/her own expense) in amounts which meet at least the minimum liability coverage limits required by the state of their residence. Clearance of driving record check; Driving records shall not contain any of the following: a suspended or revoked driver’s license, 3 or more moving violations in the past 36 months, 2 or more accidents in the past 36 months, 1 or more instances of driving under the influence (DUI) or driving while intoxicated (DWI) within the past 24 months, at fault in a fatal accident within the past 5 years, leaving the scene of an accident within the past 36 months, and/or reckless driving within the past 12 months.
PHYSICAL DEMANDS:
Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 20 pounds is required. Must be able to work with a moderate level of noise.
