About the Role
You will work across St Vincent’s Footbridge Community Care Unit, providing a high standard of care, working within the Strengths model of recovery orientated practice. You will work closely as part of the multidisciplinary team, to achieve a positive outcome for all residents and their families.
- Provide direct clinical services that meet the needs and preferences of our consumers, their families and carers

Classifications for this position will range from NP22 - NP70 ($72,404 - $96,808 per annum pro rata)
Your Contribution
- Diploma or Degree (minimum) in nursing which meets the registration requirements of the Nursing and Midwifery Board of Australia (AHPRA).
- Current Nursing and Midwifery Board of Australia (AHPRA) registration.
- A current Victorian driver’s license.
- Evidence of knowledge, skill and professional development in the area of adult mental health recovery orientated practice.
- Post qualification experience within Mental Health Services.
- A valid Nationally Coordinated Criminal History Check, or willing to obtain

About Footbridge CCU
Footbridge Community Care Unit (CCU), through a multidisciplinary team approach, provides ongoing specialist treatment and support to meet the identified needs of residents who suffer with a serious mental illness, and significant levels of psycho-social complexities in the City of Yarra and Boroondara.
Footbridge CCU is a 20 bed service which operates under a framework for recovery orientated practice, utilizing the strengths model. It is a 24 hour residential mental health service.
We support individuals with their recovery and rehab needs, help to maximize support services such as NDIS packages, and support complex discharge planning for consumers.

2.LOCAL WORK ENVIRONMENT
St. Vincent’s Mental Health (SVMH) includes an adult area mental health service for the inner city areas of Yarra and Boroondara. The adult mental health service components are: Primary Intervention and Care, the Footbridge Community Care Unit (CCU), the North Fitzroy Prevention and Recovery Care service (PARC), the 44 bed Acute Inpatient Service (AIS) located on the St. Vincent’s Hospital site, and two community mental health services (CMHS) at Hawthorn and East Melbourne (Clarendon CMHS). Community mental health services comprise the Triage and Crisis assessment and Treatment (CAT) service, Extended Triage and MH-HARP, Continuing Care (CCT), Mobile Support and Treatment (MST) and Homeless Outreach (CHOPS) services. A Consultation & Liaison Service is provided to the health service. NEXUS Dual Diagnosis Service, the Body Image and Eating Disorders Assessment and Treatment Service (BETRS) and NEVIL Training Cluster are regional services. Statewide services are the Victorian Dual Disability Service, the Victorian Transcultural Mental Health and Dual Diagnosis Education and Training Unit. The Acute Inpatient Service includes 5 beds designated as the Koori State-wide Inpatient Service and linked to the Victorian Aboriginal Health Service.
All St Vincent’s Mental Health clinical staff are expected to be familiar with, and practice in a manner consistent with the National Practice Standards for the Mental Health Workforce.
3.POSITION PURPOSE
Footbridge CCU is a 20 bed community residential service for consumers with a mental illness and complex psycho-social issues. The service consists of a mixture of single units and shared units. We operate on the principles of recovery orientated practice, utilising the strengths model.
There are two streams provided by Footbridge CCU, the Recovery Review Program (RRP) and the Intensive Recovery Review Program (IRP). The RRP is a 6 week comprehensive multi-discplinary assessment, out of which recommendations are made about the most suitable care and support options. Where a stay at Footbridge is recommended, the consumer is invited into the IRP.
The IRP is an intensive program focussing on the individual’s recovery goals, discharge planning and future supports required. This is done through case management, utilising the skills of allied health and nursing staff.
Under the guidance of the ANUMs/NUM at Footbridge CCU you will undertake RRP assessments, ensuring that best practice standards are met and maintained. You will also require to undertake case management and associate case management of consumers.
Multi-disciplinanry liaison and working with families and carers is a key component of the role, as well as inter-agency liaison.
4.POSITION DUTIES
- Provide clinical services to consumers and carers including the provision of high quality mental health assessments, case management and clinical interventions.
- Contribute to continuous quality improvement activities within the service.
- Maintenance of all documentation in line with SVMH policies.
- Attend and contribute to all clinical and non-clinical meetings at Footbridge CCU.
- Undertake consumer assessments under the RRP.
- Hold a designated port folio, with support.
- Administer prescribed medications.
5.INCUMBENT OBLIGATIONS
General
- Perform duties of the position to best of their ability and to a standard acceptable to SVHM

6.SELECTION CRITERIA
- Demonstrated ability and knowledge of a recovery oriented, community based psychosocial approach promoting social connectedness and inclusion.
- Well-developed ability to develop care plans that are creative and flexible, place the consumer as the expert and leader of their care, and which draw in services from across a range of sectors.
- Demonstrated ability to engage with, and work collaboratively with people with severe mental illness and complex psychosocial needs.
- High level communication skills, written and verbal.
- Advanced computer skills, including Microsoft Office applications
- Demonstrated planning and organisation skills and an ability to work independently
- Diploma or Degree (minimum) in nursing which meets the registration requirements of the Nursing and Midwifery Board of Australia (AHPRA)

7.1 ESSENTIAL REGISTRATION, LICENSE OR QUALIFICATION REQUIREMENTS
- A current Victorian driver’s licence.
7.2 OTHER ESSENTIAL REQUIREMENTS
- Clinical assessment and care planning skills.
- Experience in the provision of assessment, discipline specific intervention and case management services to consumers with a serious mental illness and complex needs and their carers within a recovery framework.
- Demonstrated ability in working as part of the multi-disciplinary clinical team in promoting a standard of excellence in clinical practice. This includes contributing to team clinical processes such as Clinical Review and communicating effectively with other disciplines.
- Knowledge of the Mental Health and Wellbeing Act 2022
- Commitment to working collaboratively and effectively as part of the multidisciplinary team with multiple key stakeholders both internal and external of the St Vincent’s Health organization (including consumers, carers, family members and community agencies).
- Demonstrated ability to working collaboratively with consumers and carers to promote self determination and enable progress towards reaching personal goals.
- Highly developed interpersonal and communication skills, including problem solving ability and conflict resolution skills, as well as sound oral and writing skills.
7.3 OTHER NON-ESSENTIAL REQUIREMENTS
- Knowledge and experience of AOD and harm reduction would be an advantage.

National Police Check:
I understand that it is a condition of my employment to provide SVHM with a current National Police Certificate PRIOR TO COMMENCING WORK and this is at my own cost.
I understand that regardless of the frequency, if I am working and or visiting in a designated ‘high risk area’ of SVHM (as defined in the SVHA Pre-employment/Appointment Safety Checks Policy) I will be subject to periodic Police Checks every three years at my own cost.
NDIS Clearance (if applicable):
If you are working in a designated ‘Risk Assessed Role’ (as defined by the National Disability Insurance Scheme NDIS) regardless of frequency, you will be subject to periodic NDIS Worker Screening Checks every five years at your own cost. ‘Risk Assessed Roles’ are defined as (a) key personnel as defined in the National Disability Insurance Scheme Act 2013; (b) any role that directly delivers a set of specified supports or services in the NDIS (Practice Standards – Worker Screening) Rules 2018; (c) any role where normal duties are likely to require ‘more than incidental contact’ with people with disability.
