We make everyday care for kids and adults easy, from annual checkups, vaccinations, and sports physicals to treatment for common illnesses, mental health care, women’s health care, Gender Care, and more. For extra convenience, we offer an onsite Fairview pharmacy, which means you can easily pick up prescriptions following an appointment. We are a Certified Health Care Home and offer enhanced care coordination services. Along with our physicians, we use a team of educators, pharmacists, and psychologists to teach patients how to prevent and manage disease.
Lead Registered Nurse Overview
Key responsibilities include but are not limited to:
- Works closely with Clinic Management to provide excellent quality of care for patients while maintaining good patient flow through the clinic
- Coordinates care across the health care continuum within designated sub-specialty population(s)
- Serves as single point of contact by facilitating and overseeing transitions in care within and between care settings, providers, and services
- Assesses patient needs and identifies expected nursing outcomes
- Implements nursing interventions based on plan of care
- Evaluates care and the patient’s response to interventions and expected outcomes
- Delivers care as delegated by physician/authorized practitioner to assigned patient population, or individual patient
- Uses communication strategies to achieve desirable outcomes
- Demonstrates ability to provide care or service adjusting approaches to reflect developmental level of population served (If applicable to position)
Minimum Qualifications:
- Current Minnesota RN License
- Required to have 3 years of RN experience in clinical practice
- Excellent communication skills, ability to multi-task on an ongoing basis, good organization and prioritization skills
- Must be well organized and able to build positive relationships, superior patient advocate, an effective systems user, self-motivated problem solver, continuous improver and technically advanced in nursing job role

Compensation:
