Access DH Transfer CenterJob SummaryUnder minimal supervision, facilitates the coordination of patient management for elective, direct, urgent and emergent patient flow in and out of the health system. Initiates appropriate order sets and pathways based on admission diagnosis. Arranges MD to MD consults, admission through patient tracking system, bed assignment, transportation, RN to RN reports and ED/OR notification as needed. Under minimal supervision, provide telephonic case management for members of a Managed Care Organization to include care coordination triage and utilization management admission orders for bed placement repatriation to Denver Health. Uses the nursing process to assess the patient's needs and identify appropriate resources necessary to provide the best options for the patient's situation. Provide essential communication and collaborate among all disciplines of the health care team. Responsible for the daily management of on call schedule changes for essential services provided in the health system
Essential Functions:
- Facilitates the process of acceptance and transfer of external patients from multiple sources to ensure appropriate course of action. (40%)
- Triage all Managed Care notification of patients in out of network hospitals. (10%)
- Relays findings to multidisciplinary teams such as ADU and Hospitalists Admission Team. (5%)
- Facilitates effective and appropriate use of healthcare resources for patients with complex medical and/or psychosocial needs. Understands members’ eligibility and benefit coverage. (5%)
- Review Clinical Notes and Case Presentation. (5%)
- Facilitate and manage all outreach trauma patients into Denver Health including follow up information for Trauma Services. (5%)
- Coordinate with House Supervisor for a timely transfer and appropriate bed assignment. (5%)
- Collaborate with all Critical Care, ALS, BLS transport agencies to coordinate transition of patients and resources within the continuum of care. (5%)
- Acts a resource for any related departments regarding problem solving, conflict resolution and policy interpretation to ensure smooth operation of all services. (5%)
- Assists nursing and physician staff in assessing patient condition, needs and family considerations for patient bed assignments. (5%)
- Serves as a resource person for other staff to access when questions arise regarding intake information, scheduling conflicts, information/troubleshooting and difficult patient access issues. (5%)
- Participates in the call review process of peers, and periodic evaluation of peers. (5%)
Education:
- Associate's Degree Completion of a nursing education program that satisfied the licensing requirements of the Colorado State Board of Nursing for Registered Nurses. Required
Work Experience:
- 1-3 years trauma and/or emergency care experience Required
Licenses:
- BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association or American Red Cross Required and
- RN-Registered Nurse - DORA - Department of Regulatory Agencies Required
Knowledge, Skills and Abilities:
- High organizational and interpersonal communication skills.
- Possess excellent customer service standards and a customer-friendly voice.
- Able to critically think and multi-task effectively, sometimes in a high stress situation.
- Understands admission procedures, EMTALA and insurance vetting.
- Be proficient with typing and working with electronic medical record systems and operate multiple phone line system.
- Be proficient with insurance standards. Demonstrates expertise in use of Patient Tracking System.
- Demonstrated and understanding of assessment findings as they relate to disease processes and continuum of care.
- Ability to follows all protocols and standard procedures for digital recorded calls.
- Ability to sort through all patient information and formulate an accurate picture of the patient and his/her condition for the MD’s and Nurses.
- Ability to use computers and computer systems.
- Bilingual skills preferred
